This position will cover hospital locations in Essex County and support the Northern Hospital Division team during on-call hours. Responsibilities: Complete evaluations, admissions, and discharges within our Northern Hospital Division as needed. Medicare-eligibility review for new referrals and determining if patients meet criteria for GIP-level hospice. Follow up on current GIP hospice patients to ensure needs are met during on call hours. Reinforce hospice recommendations made by RN Case Managers and make adjustments as needed. Collaborate with doctors, nurses, social workers, patients, families, and our hospice staff to promote exceptional end-of-life care. Be available to answer any questions and provide education as needed regarding hospice services and philosophy.
