Performs a comprehensive patient assessment, including the collection of relevant medical data. Documents in the medical record include relevant assessment data, educational plan, education services provided, evaluation results, and relevant discharge/support services recommended or coordinated. Documents community diabetes education activities performed as per established processes. Evaluates the effectiveness of DSME services provided by measuring attainment of learning objectives, through patient/family demonstration and/or verbalization. Participate in continuous quality improvement processes to measure the DSME process and identify and address opportunities for improvement within Parkland and/or community-based activities. Collaboratively (patient, multidisciplinary team, and referring provider) develops educational goals, learning objectives, educational content, and teaching methods, including an individualized support plan, for all patients referred for DSME within Parkland and, where relevant, for diabetes education activities conducted in the community. Provides educational interventions that utilize interactive, collaborative, skill-based training methods. Dietitian Certified Diabetes Care and Education Specialists can undertake all steps above EXCEPT administering a patient's injection. Liaises with the multidisciplinary diabetes care team, including clinical, business, and relevant community partners/support personnel, to optimize patient diabetes self-management. Serves as a patient/community advocate by focusing on their needs, rights, confidentiality, and religious and cultural preferences. Ensures positive working relationships are maintained with all Parkland internal and external customers, employees, and community partners, etc. Serves as a resource for patients, physicians, other healthcare team members, and community partners/support personnel. Participates in the ongoing development of health professional, patient-centered, and community education programs and resources (live, print, online, other) to increase Parkland's capacity for optimal diabetes care delivery, reduce clinical incidences, and attain desired patient, health system, and community outcomes for diabetes. Within the multi-disciplinary framework of diabetes management and education, it identifies opportunities for providing and receiving peer education and direction. Strives to advocate for and implement the American Diabetes Association Standards of Medical Care in Diabetes and the National Standards for Diabetes Self-Management Education and Support to assist in building the structure to deliver consistent, best-practice care across the health system and community to achieve better outcomes. May lead and/or participate in community outreach activities (such as health fairs, community collaborations, or outreach clinics) as assigned. Serves on diabetes-related committees and completes special projects, including possible community projects, as assigned. (Dietitian) Screens patients for nutritional risk, when applicable. Upon Hire (ACLS) ADVANCED CARDIAC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Job Duties Develop and implement an effective action plan to improve performance in the department. Meets or exceeds financial goals and objectives of the department. Provides direct patient care and/or functions as "charge" position as needed. Seeks out opportunities to teach and mentor other nursing personnel on formal and/or informal basis. Evaluates and supervises provision of patient care by other personnel assigned to unit. Adheres and facilitates adherence by other personnel to all policies and procedures, code of ethics, JCAHO standards, and nationally recognized standards of nursing practice. Reviews nursing assignments and revises as necessary to meet specific needs of patients and/or to ensure adherence to policies and procedures. Implements disciplinary interventions, including termination, as appropriate and in compliance with policy. Participates in identification and resolution of physician, patient and/or family complaints. Reports complaints and interventions to Guest Services and/or Department of Nursing in timely manner for adequate follow-up prior to patient's discharge. Reviews schedule and staffing plan with other Department of Nursing personnel to ensure adequate coverage of patient care needs on daily shift or shift-by-shift basis. Utilizes staffing guidelines and/or patient acuity system and employs most cost effective option in providing appropriate patient care. Utilizes resources in manner which is appropriate and which optimizes resources for benefit of organization and patient population. Identifies efficiency or cost saving ideas and reports same to DON and/or CNO. Interviews and makes final decision regarding hiring of all nursing personnel for unit. Implements disciplinary interventions, including termination, as appropriate and in compliance with policy. Participates in development, implementation, and evaluation of performance improvement activities. Completes performance reviews and orientation evaluations of nursing personnel. Implements termination of nursing personnel with approval from Chief Nursing Officer and/or Human Resources Manager. Provides immediate counseling of nursing personnel, up to and including suspension, as appropriate. Follows-up with accurate and timely written reports to Nursing and/or Human Resources Director. Generates and/or supervises generated occurrence reports as appropriate. Provides appropriate and timely follow-up and submits to DON. Conducts staff meetings and coordinates special projects, which may include educational activities, research projects, recruitment, and interdisciplinary and hospital committees. Collaborates with nursing staff and Nurse Educator to meet educational needs of current nursing personnel. Promotes personal and professional growth of all nursing personnel by allowing opportunities for active participation in unit-based activities and appropriate patient care. Seeks out opportunities to teach. Develops unit's annual capital and operational budgets with assistance from nursing administration personnel. Completes payroll editing at end of payroll cycle and maintains attendance records. Provides and supports efforts to develop teamwork between all nursing personnel, employees and staff. Facilitates timely return of specialty equipment. Participates in routine and special care conferences. Demonstrates ability to function in "charge" role. Performs other duties as assigned.
