POSITION OVERVIEW The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit. Key Responsibilities Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition. Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician. Provide holistic care for residents within the scope of your LPN license. Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents. Grand Rapids, MI 49525. This is our primary location where we see a mix of adult patients Monday through Friday, 8am – 5pm. The providers practice out of the Clear Vista, Grand Haven Center, Big Rapids locations. There is currently one physician assistant that see patients at the Grand Haven Center in Grand Haven, MI 3 days/week. There is currently one physician that sees patients two days a week in Big Rapids and a physician assistant that will be seeing patients two days a week in Big Rapids soon. We have a team of 7 physicians, 3 advanced practice providers, nurses, medical assistants, and ambulatory care technicians. This MA would work in tandem with our nurses and providers to support our patients clinically. The MA will assist with patient rooming, medication injections, ultrasound guided injections, vaccine administration, along with chart prepping, patient phone calls, patient MyChart messages, electronic faxes, medication authorizations, and quality assurance tasks. There is an expectation, though very rare, that travel to the Grand Haven and Big Rapids locations would occur if there were staffing needs however, we currently have clinical team members that primarily support the Grand Haven and Big Rapids clinics. General Purpose of Job : The RN Staff Nurse is a professional care giver who assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the environment / instrumentation, and other health care team members. About the Practice This position will support our Occupational Health Clinics located at 426 Michigan, 4600 Breton and occasionally our Zeeland location. Keeps exam room stocked with adequate medical supplies, maintains instruments and prepares them for sterilization and storage. Provides clerical support which may include completing forms/requisitions, answering phones, sending/receiving medical records, obtaining lab/x-ray reports, hospital notes, and referral information. Cleans rooms between patient visits in accordance with infection prevention and control standards. Administers medications and immunizations under authorization and supervision of physicians and/or advance practice providers. Processes messages from patients and front office staff to physicians and advance practice providers. Processes messages and call backs to patients, medical offices and pharmacies. Processes medication refills according to protocols. Maintains all logs and required checks (i.e. refrigerator temperature, emergency medications, expired medications, oxygen, sterilization fluid charge etc.) Performs EKG’s and simple (CLIA waived testing) medical office laboratory and screening tests. Collects and processes blood samples and other non-blood specimens for the diagnosis, treatment and prevention of disease.
