Description The Sepsis Quality Program Manager (QPM) is accountable for the overall management, leadership, and direction of the clinical and operational activities of the Sepsis program at Swedish campuses. The Sepsis QPM RN will work, across Swedish campuses, to create an environment that fosters a multidisciplinary and collaborative team approach to care, treatment, and follow-up for septic patients that achieves operational and clinical "Best Practice." The Sepsis QPM RN works with campus directors, unit managers, and other leadership teams to oversee the daily operations of the sepsis program initiatives and assists nursing, medical, and ancillary staff in providing quality care outcomes for the care of patients with sepsis. The Sepsis QPM RN achieves and maintains excellent performance and tracks and reports impact of sepsis care delivery on patients' quality of life; always striving to improve, while acting in partnership with all departments. Job Description UW Medical Center - Montlake has an outstanding opportunity for a Post Liver Transplant Registered Nurse to join our Transplant Services team. The focuses on the duties related to the post-transplantation phase and will include procurement call. Management for the designated transplant patient population includes independent patient assessment, triage, education, coordination of medical and nursing care, and documentation of care. Integral to the role is an understanding of the regulations and data requirements surrounding transplant care issued from the United Network of Organ Sharing and HRSA. Job Description Responsible for the nursing care and coordination of the post liver transplant long-term patients, plus the surgical (non-transplant) patient education and surgical prep, as well as multiple clinical procedures on a daily basis. Nursing Process – Systematically assesses the need for and the delivery of nursing services that meet patient care requirements, whether delivered directly or by the other nursing staff.
