Individualize the plan of care and make adjustments as needs change. Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs.
