Provides clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff in completing nursing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals; advocates for individuals’ needs; and may provide instruction to providers on individuals’ discharge plans. Overview Coordinates and provides high quality multidisciplinary health services for patients in the Hospice Care Program consistent with Agency philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. SCHEDULE: Friday, Saturday, Sunday 2:00pm-2:00am • Assesses the physical, social and psychological needs and living conditions of the patient through professional knowledge and skills of observation, interviewing and physical examination. • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. • Implements care plan and revisions by ongoing assessment, planning, coordination, counseling, teaching and supervision. Works collaboratively and consults with appropriate multidisciplinary health team members (i.e., physician, OT, PT, SW, SLP, family members). • Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes and care facilities. • Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice. This may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care. • Documents content and process of all services provided in accordance with government regulations and requirements, Agency policies, practices, and procedures. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • Monitors the effect of the care plan on patient and family health status and informs physician and other team members of changes in the client's needs. • Consults with and educates the client and family regarding the disease process, self-care techniques and prevention strategies. • Appraises care provided and revises the goals and approaches to patient care, as needed. • Initiates and maintains written (including clinical and progress notes) and verbal communications with team members and management to assure optimal patient care. • Performs established work procedures in relation to Program workflow. • Understands, interprets and implements the Agency standard reimbursement policy and determines the billing source for patient care. We are currently seeking CNAs who will embody the passion and commitment required to deliver the ultimate care for our residents. All positions are scheduled to work every other weekend (Saturday and Sunday) . part-time all shifts (7:00am-3:15pm, 3:00pm-11:15pm and 11:00pm-7:15am) EDUCATION AND EXPERIENCE REQUIREMENTS : •Current certification in New York State Nurse Aide Registry (not expired or lapsed). •No findings or convictions of resident abuse, mistreatment, exploitation, neglect, or misappropriation of resident property listed on the Registry against them. •One year long-term care experience and CPR certification preferred. DUTIES AND RESPONSIBILITIES : Performs a variety of services for geriatric residents under the direction and supervision of a licensed nurse: 1. Assists with personal care services and Activities of Daily Living for residents under the direction and supervision of a licensed nurse, including: * Bed baths, showers, tub baths and shampoos, perineal care, oral hygiene, hair and nail care (non-diabetic), and any other personal hygiene or resident care ordered/required for the wellbeing of the resident. * Toileting - including understanding and applying knowledge of bowel and bladder training regimens, gives and removes bed pans and urinals as needed, distributes drinking water and nourishments, assists with resident's meals including transporting to dining room, distribution and collection of trays and feeding residents. Routine am and pm care. 2. As assigned by the team leader or charge nurse, the CNA is responsible for the following related duties: * Maintaining non-patient areas (i.e. tub room, utility room, etc.) in a neat and orderly condition, as assigned. o Assisting with admission and discharge procedures. * Answering residents' call lights. * Performing errand services for nursing unit as needed. * Maintaining resident's living area, consisting of bed, bedside table, overhead table, wheelchairs, geriatric chair clothing drawers and closets and bathroom storage area in a clean, orderly manner. 3. Observes and reports information related to changes in patient's condition or patient's responses to care to the team leader or charge nurse including: * Measuring and recording fluid intake and output. * Taking and recording temperatures, pulses and respiration's. * Obtaining routine specimens: urine, stool and sputum. * Testing urine for sugar and acetone. * Administering routine enemas. * Obtaining and recording resident's weights. * Assisting residents in and out of bed, chairs and stretches; lifting, turning and positioning residents, applying principles of good body alignment.
