Job Requirements Job Knowledge and Skills Basic Cardiac Life Support (BLS) required Basic Dysrhythmia preferred or obtained prior to completion of orientation Crisis Prevention Institute (CPI) training preferred or obtained prior to completion of orientation Education and Experience Current New Hampshire RN license Completion of the Nursing Residency Program within 18 months of hire. This program consists of a unit specific nursing orientation full time, in addition to one year classroom/SIMS education, and extended mentorship. The Regional Director of Resident Services is responsible for evaluating standards of quality, service, training, expense control, and revenue generation within our communities. The role is one of support for the nursing and executive leadership within each community. With a focused portfolio of five Massachusetts communities — including one skilled nursing community — each fully staffed without reliance on agency staffing, and well-established clinical support at both the community and corporate levels, this role offers regional impact with a manageable, sustainable scope. Essential Functions Regulatory and Corporate Compliance: Ensure each community within the defined region is compliant with Maplewood policies, procedures, practices and programmatic standards, as well as with Massachusetts Executive Office of Aging & Independence (AGE) regulations related to resident care and clinical operations, and applicable federal/CMS requirements for the skilled nursing setting. Participate in team meetings to ensure effective communication among all disciplines involved in participant’s care. Follow the CBHC participant across the continuum of care to ensure care coordination. As well as important information related to interactions with the patient and/or visitors. Reports any unusual or changes in patient condition to the primary RN and/or charge RN Completes bedside shift hand-off report routinely and independently. Answers call lights to determine patients’ needs in a timely manner. Understands and demonstrates initiatives for improved response times to our patient’s needs such as NO PASS ZONE. Performs bladder scans, EKG’s, CPM’s, assists with cold therapy and other appropriate medical procedures upon completion of appropriate competencies. Bathes patients daily and assists with personal hygiene to increase their comfort and wellbeing. Assists with performing in the Hospital Unit Coordinator role as necessary to meet the needs of the unit. Prepares equipment and assists other nursing staff during treatments and examination of patients. Uses equipment/safety protocols and policies in providing care and transport to patients. Demonstrates understanding of the fall prevention measures for all patients assessed by the RN as high fall risk. Communicates with the RN and provides report to on coming LNA any concerns. Utilizes AIDET when interacting with patients and assists with providing hourly rounding to all patients. Completes all assigned annual competencies by due dates. Foundation Medical Partners, our multi-specialty group, spans 70+ practices across southern New Hampshire and northern Massachusetts, providing coordinated, patient-centered care to thousands each year. About the Job: As a Registered Nurse Ambulatory, you’ll provide professional nursing care across our Foundation practices, including float pool support. Working collaboratively with providers and the care team, you will deliver safe, evidence-based, patient-centered care both in-person and through telephone triage and telehealth modalities. What You’ll Do: Carry out the nursing process—assessment, planning, intervention, and evaluation—across ambulatory and telehealth settings. Collaborate with physicians, advanced practice providers, and interdisciplinary team members to design and implement plans of care. Administer medications, vaccines, treatments, and perform point-of-care testing within scope of practice. Educate patients on self-management, medications, nutrition, health, and wellness. Document care accurately in the EMR and participate in pre-visit planning for patient encounters. Provide leadership and mentorship to clinical staff and assist with onboarding. Respond to urgent or emergent situations in accordance with facility guidelines. Stay current on competencies, licensure, and ongoing educational requirements. Who You Are: Associate of Science in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred. Current Registered Nurse license in NH (or NLC state/MA) and BLS certification required. 1+ year of professional nursing experience preferred; ambulatory care experience a plus. Strong knowledge of nursing principles, EMR systems, and patient-centered care models. Skilled communicator with excellent time management, adaptability, and teamwork abilities. Compassionate, detail-oriented, and able to thrive in a fast-paced, collaborative environment.
