The LPN MDS Coordinator gathers information, assesses needs, establishes reasonable goals, provides interventions and incorporates within an organized, concise, functional care plan. Coordinates completion of comprehensive assessment by interdisciplinary team and includes recommendations in the written care plan for each resident. Each plan must identify all relevant issues for the care of the resident as well as the goals to be accomplished for each problem or need identified. The LPN MDS Coordinator works together with care planning team to implement final plans. Together with the Nursing Director, the RN Registered Nurse Shift Supervisor plans the operation of nursing services and the provision of emergency support requirements for tour of duty. Plans the coordination of nursing activities for tour of duty in line with medical needs, physician instructions and the general nursing care plan. Coordinates the activities of charge nurses to ensure proper care of residents. In conjunction with medical staff, adjusts the care plan to meet emergency medical needs. In the absence of superiors, the RN Registered Nurse Shift Supervisor assumes total responsibility for operating decisions required by the circumstances of the moment. Takes responsibility for management of emergency situations as the need develops. Ensures presence of appropriate relief staff to assume responsibility prior to leaving the facility. Ensures notification of resident’s personal physician if there is a significant change in the condition of the resident or if the resident requires immediate medical care. Required Qualifications Must hold a current state RN license and be a nurse in good standing. Must meet all applicable federal and state licensure requirements. Ability to supervise and instruct others. Preferred Qualifications Attention to detail, good follow through skills and ability to prioritize multiple tasks. Previous experience in a hospital or long term care facility.
