Location: Lodi, NJ (Bergen County) Position: Part-Time (3 hours/day, 7 days a week) Schedule: 8:00 AM - 11:00 AM monday-sunday (must be available all 7 days) Pay: $19 per hour Job Description: We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to provide essential care for a male patient at his home in Lodi, NJ. The patient, who has been in a nursing home but wishes to be cared for at home, has specific needs due to a recent heart attack and ongoing health issues. Responsibilities: Administer feeding through a feeding tube. Change diapers and assist with all aspects of personal hygiene. Assist with washing down the patient. Ensure the patient’s comfort and safety while in bed. Provide compassionate companionship and support. Requirements: Certification as a Home Health Aide (CHHA) in New Jersey. Experience with feeding tubes and personal care. Ability to handle physical demands, including changing diapers and assisting with hygiene. Reliable, punctual, and able to work 7 days a week. Excellent communication skills and a caring attitude. Additional Information: The patient’s wife is retired and at home, available to provide support and assistance. The position is part-time, requiring 3 hours of care per day, from 8:00 AM to 11:00 AM. Gender of the caregiver does not matter. Position Summary To fulfill the needs of patients in a variety of age groups, their family members, and the community by performing varied patient care tasks and functions in accordance with licensure, certification, training, and regulatory requirements. Manual and finger dexterity and hand-eye coordination are also required as is constant talking and listening in English. Must demonstrate effective oral and written communication skills, including speech clarity, understanding the information relayed, deductive and inductive reasoning skills (the ability to apply general rules to specific problems to produce answers that make sense and the ability to combine pieces of information to/from general rules or conclusion respectively). Considerable walking, standing, sitting, reaching, filing, typing and photocopying required. Ability to perform work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and changing work environment related to changing needs of patients with acute, chronic and complex and/or terminal disease processes. Ability to work cooperatively within health system, multidisciplinary team members (i.e. MD's, Rehab, Respiratory, DI, Laboratory, etc.). Ability to utilize effective time management skills to set priorities to perform responsibilities, problem solve and deliver care. Collaborates with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team; collaborates with the appropriate physician on patient’s plan of care. Accurately interprets and implements treatment regimen as prescribed by Physician. Assists Physician during treatments and/or diagnostic procedures. Keeps Patient Care Manager/designee and/or physician, abreast of changes in patient’s condition and/or treatments, as appropriate. Uses clinical judgment in delegating assignments in providing patient care, and ensures assignments are completed in a timely fashion. Performs grade I-IV Decubitus Care; performs preventive skin care measures. Applies simple dressings, maintaining principles of aseptic technique. Applies warm and cold compresses, consistent with facility procedure. Performs irrigations, consistent with facility procedure. Performs other procedures related to skin care, as necessary. Administers medications correctly and safely; correctly identifies medication in terms of action, dosage, side effects, and implications for the patient. Meets standard on medication administration examination. Demonstrates preparation of local solutions. Administers and documents medication correctly. Educates patients and significant others related to drug and food interactions. Participates in patient and family education. Provides patient with an explanation of his/her condition as indicated. Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies. Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching. Provides for early discharge planning and appropriate referrals for post-hospital care. Evaluates effectiveness of teaching by feedback from patient/family and documents same. Performs related duties as required.
