The Mobile Hospital at Home Registered Nurse provides hospital-level acute care in a patient’s home within a defined radius around designated hospitals as part of the Advocate Hospital at Home team, which may include the following: Physicians, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Physical Therapists, and other operational staff according to the Hospital at Home Care Model. Ability to work independently while in the patient’s home in collaboration with the virtual care team. Provides comprehensive nursing assessment in an untraditional nursing environment. Physical Requirements and Working Conditions Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday in possible confined spaces. What You’ll Do: Uses the nursing process to assess the needs of the patient, plan and implement individualized interventions in the home and evaluate the effectiveness of the plan of care. Includes the patient and family in planning and implementing care and develops and maintains a therapeutic relationship throughout the healthcare continuum. Anticipates and recognizes significant changes in the patient’s condition, status, and need for care escalation that may be need in the in-home care setting. Provides in home assessment of the patient and takes appropriate actions when changes occur. Anticipates and alters the plan of care as needed, mobilizes appropriate resources, and collaborates with other health care team members to coordinate patient centered care. Implements strategies to reduce patient risk and increase patient safety while in the home. Actively coordinates the discharge plan utilizing an interdisciplinary approach. Applies evidence-based practice to deliver patient care in the home. Consistently incorporates nursing and regulatory standards of care into practice. Provides ongoing in-home evaluation of patient, team, and unit outcomes and is empowered to take appropriate action when changes are needed while providing care in the home. Supports shared governance activities and initiatives to improve processes and patient outcomes. Demonstrates effective communication and promotes collaboration among healthcare team members virtually and in-person. Assumes a leadership role in the care team and delegates appropriately to unlicensed personnel. Promotes a culture of safety through identifying threats to patient and in-home care team’s safety and intervening to prevent harm. Reports patient safety events and near misses in a timely manner. Seeks to identify potential safety issues and assists in the implementation of corrective actions. Assistant Director of Nursing RN Duties and Responsibilities: Reviews daily work assignments to ensure they meet regulatory compliance and provide consistent staffing whenever possible. Assists DON in daily managing of the nursing staff, scheduling breaks, reassigning employees from one area to another area as facility needs dictate, and enforcing policies with the authority to issue Coaching and Corrective Action with HR/Administrator guidance. Participates with the Medical Consultants and the Administrator in the selection of all potential new admissions to the facility and assess items needed in the facility to meet the new resident’s needs, admission, retention, and discharge of residents. Ensures physicians make visits as required. Ensures necessary equipment, supplies, and staffing are available for the delivery of the prescribed resident/patient care. Manages the nursing department budget and provides Administrator with budgetary planning information related to equipment needs and staffing. Collaborates with the DON and Administrator in the planning of staff development programs that will enhance staff knowledge in providing quality resident care. Works with regional personnel to continuously improve the quality of resident/patient care. Strategic plans or program development for improved outcomes to reduce unnecessary returns to acute care. Ensures that all elements of the facility’s Abuse & Neglect policies are maintained and enforced. Perform other duties as assigned. Assistant Director of Nursing RN Skills and Abilities: Proven strong management skills, including delegating responsibility to subordinates, performance management, conflict management, and leading through others. Must have good financial and management skills. Must be able to adapt to a flexible work schedule and occasional overnight travel. Ability to read, write, speak, and understand English. Meets all health requirements, as required by law. Demonstrates basic computer knowledge and ability with an aptitude to learn company applications as well as proficiency with Microsoft Office Suite (Excel, Word, Outlook, PowerPoint). Assistant Director of Nursing RN Education and Experience: Current CPR Certification required. Bachelor’s or Associate’s degree from an accredited School of Nursing. Current RN license with the state required. Prefer previous nursing and leadership experience of at least three years; ADON preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required. Travel up to 10%. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
