Prepares patient for examination including patient assessment, histories, vital signs, medication reconciliation, and routine testing of hearing, vision or other. Document pertinent information in chart. Maintains documentation standards and reviews chart prior to appointment to assure requests and documents are in place. Administer treatments as requested by the physician. Obtain specimens for urine testing, phlebotomy, etc. Performs laboratory tests as directed by the physician. Accurately documents required information in patient charts. Set up and assist physician with procedures (laceration repair, pap/pelvic, etc.). Schedule outside tests or procedures for patients in other departments of facilities such as x-ray, MRI, etc. Prepares patients for tests/procedures by providing appropriate instructions. Coordinate admissions, referrals, call in prescriptions, etc. Stock exam rooms with supplies as needed. Conduct day to day inventory of supplies and report items to be ordered. Assist in the scheduling of appointments in EMR system as required. Able to screen calls and determine urgency, necessity of appointments or whether phone advice is sufficient. Position Summary: The Certified Nursing Assistant provides quality nursing care to residents. They coordinate work within the department, as well as with other departments, report pertinent information to the immediate supervisor, and assists immediate supervisor with tasks to support department operations. Assists in maintaining a safe, neat and clean environment; reports environmental deficiencies to the Environment Plant Manager such as lighting or equipment problems. Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items. Observes residents for changes in condition or behavior and promptly reports these changes to the Charge Nurse. Provides care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas. Answers residents’ call lights promptly and courteously. Performs various tasks assigned by the charge nurse, including checking vital signs, weighing residents, applying creams/ointments and collecting specimens. Assists with orienting residents and their families to the nursing home on admission and to the unit when transfers occur. Lifts, moves, and transports residents, using proper body mechanics or lifting devices for accident prevention. Also use wheelchairs, slide boards, walkers, and canes as required. Communicates and interacts effectively and tactfully with the residents, visitors, families, peers and supervisors. Assists and escorts residents for appointments such as at the beauty shop or attending activities or church services. Participates in activities and functions as directed. Completes documentation of care provided or other information in PCC in accordance with department policies. Required/Desired Qualifications: Education, Training, and Experience: Must possess a current Michigan Certified Nurse Aide Certification or have completed a Michigan approved CNA training program and are eligible to test. This is an annual CBL that describes the difference in a swing bed patient versus an in-patient or observation patient. Bronson South Haven Hospital and Bronson Lakeview Hospital employees who are approved to work in a Swing Bed Facility only: Employees may be expected to cover shifts at Bronson Lakeview Hospital and/or Bronson South Haven Hospital depending on patient acuity and staffing needs. Virtual Registered Nurse (VRN) Role: The virtual registered nurse leverages technology to deliver patient care remotely, collaborating with bedside staff and the broader care team to ensure quality outcomes and timely discharge. General Summary: The Registered Nurse for the Surgical Center is a professional care giver who assumes encounter-based responsibility and accountability for surgical patients and provides care to these patients via therapeutic use of self, the nursing process, the environment / instrumentation, and other health care team members. Principal Duties and Responsibilities: Assess resident conditions to implement intervention and develop Care Plans. Accurately record resident observations in PCC and follow through on Care Plans. Assure facility medical records policies and procedures are adhered to. Make regular rounds of residents independently and with a physician; observe and evaluate resident symptoms, progress, and reactions to treatments and medications and implements interventions as necessary. Provide preventative, supportive, maintenance, and rehabilitative care directed to the physiologic and psychosocial needs and well-being of residents. Provide care to residents by performing a variety of treatments, including changing dressing, cleansing and/or irrigating wounds and incisions, performing Foley Catheter irrigations, IV therapy, and administering tube feedings, chest tubes and peritoneal dialysis. Request and assure supplies and medical equipment are available to perform care and treatments as required by the resident. Schedule appointments, physicians, therapy, and consults. Document in appointment book. Monitor and assist CNAs with personal resident care duties such as bathing, feeding and assisting with ambulation, and other routine accommodative services for resident comfort and well-being. Receive reports from outgoing nurse and make rounds to see each resident daily and as needed. Administer medications to residents according to the Public Health Code, Nursing Department Policies, and standards and procedures and as prescribed by the physician; notify appropriate clinical and/or nursing personnel of medication contraindications. Advise residents and/or responsible persons regarding changes in medications, treatment plans, and resident condition. Manage and document emergency care of residents, coordinating communications to physicians, responsible persons, DON and Administrator, hospital personnel, emergency transport, and other appropriate personnel. Perform comprehensive and focused nursing assessments in conjunction with changes in condition and scheduled programs. Admit, transfer, and discharge residents. Requests and assures supplies and equipment are available to perform care and treatment as required by the patient prior to or at the time of admission. Assure completion of weekly, periodic and discharge summaries. Perform quality improvement activities as assigned by the Director of Nursing. Monitor rate of accident/incidents occurrences and infection rate on the unit and develop suggestions for corrective action. Weekly skin assessment to monitor occurrence of pressure injuries or other skin conditions, providing the Director of Nursing written data on the number of occurrences. Report resident and staff complaint and all unusual occurrences to the Director of Nursing or Administrator and any allegation of Abuse/misappropriation or neglect to the administrator immediately. Participate in periodic regulatory surveys by the State of Michigan and participates in the preparation of the Plan of Correction response to an inspection survey. Complete and report resident injury to Unit Manager. Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrator incidents or evidence of resident abuse or violation of residents’ rights. Know and support facility philosophy, standards, policies and procedures. Communicate with department heads on an as-needed basis to problem solve and plan for more effective patient care. Inform Director of Nursing when staffing requirements are not met. Present a professional appearance and serve as unit role model. Maintain own professional requirements for continuing education and staff development by participating in work related seminars, etc. Promotes compliance with accident prevention procedures, safety rules, and safe work practices to prevent employee injury and illness and control worker’s compensation costs. Ensure equipment is used in a safe manner and report any maintenance concerns to the Environment Plant Manager. Adheres to the Nurse Practice Act. Attend staff meetings and in-services as requested. Perform other tasks as required. Required/Desired Qualifications: Education, Training, and Experience: Must possess a current Michigan license as a Registered Nurse. CPR certification required. Two years of professional nursing experience in the long-term care setting is preferred. Specific skills, knowledge, and abilities: Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a long-term care setting. Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations. Basic computer knowledge is required.
