Job Description Summary: An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home, or in an alternate infusion site, to ensure patient safety with continuity and compliance under a physician’s plan of care. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Under close supervision, coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Naven Health’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Qualifications Licensed physician, physical therapist, speech-language pathologist, occupational therapist, audiologist, social worker, registered nurse (nurse practitioner, RN or other advance practice nurse). Registered nurse is strongly preferred. • Current driver's license. • Completion of required continuing education for licensure. • Graduate of a state‑approved school. • Minimum of one year experience in home health. • Knowledge of Nursing policies and procedures; administration of medication, possible side effects, and treatment as prescribed; federal, state, and local laws and regulations relating to patient care; comprehensive knowledge of nursing practices; physical, psychological, social, and medical needs of clients; organizational structure and duties of employees under supervision of the Director of Home Health. • Strong professional conduct, emotional stability, and teamwork.
