Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21. Follows Primary Care Medical Home and MACRA protocols for designated role 22. Performs all other job related functions as assigned. Description The Offsite Medical Assistant I Provides clinical office support to Physicians & Surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities Fulfills patient care responsibilities as assigned which may include: checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring, dressing, collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. Inventories, orders or re-stocks medical supplies as applicable. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. May be required to perform proficient phlebotomy duties. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. May maintain, adjust and confirm patient office schedule. May check and verify & update insurance information utilizing internal or web sources. Description Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse. Responsibilities A. Clinical/Technical/Service 1. Participates in the application of the nursing process to an assigned group of patients. 2. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse 3. Performs job duties under supervision of a Registered Professional Nurse. 4. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. 5. Administer IV fluids, provided completion of annual competency IV therapy training. 6. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. 7. Makes recommendations in the application of the nursing process. 8. Participates in planning, and administers direct patient care, including activities of daily living (ADL). 9. Performs point of care testing including blood glucose. 10. Performs nursing procedures as defined in the nursing procedure manual. 11. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. 12. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. 13. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. 15. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. 16. Makes, reports and records observations on assigned patients within the EMR. 17. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. 18. Participates in health teaching to patients and families following the RNs development of the teaching plan. 19. Establishes an environment that promotes optimum physical and emotional comfort. 20. Performs purposeful hourly care checks and supports the patient experience in accordance with Relationship Centered Care Professional Practice Model 21. Organizes supplies and equipment necessary for carrying out the plan of care is identified and communicated by the charge nurse/designee. 22. LPNs in the Outpatient Hemodialysis Setting An LPN can practice in the chronic hemodialysis outpatient department under the direct supervision of an RN. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Description The Assistant Nurse Manager for Mount Sinai Queens Medical/Surgical unit is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Clinical Nurse Manager. Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient's condition that falls within their specialty area of practice. Responsible for assisting physicians/providers with patient care, rooming patients, and performing clerical, environmental, and organizational tasks. Identifies self to patient/family and explains to patient/family the roles of each member of the care delivery team. 1.2. Accompanies patients to the exam/procedure room. 1.3. Prepares patients for visit. Typically includes vital signs, height, weight, blood pressure, EKG, specimen collection (where applicable), phlebotomy (where applicable), INR (where applicable), pulse oximetry, spirometry, etc. Documents medical and surgical history, family history, social history, problem list, and medications in EPIC. 1.4. Performs screening per physician/provider guidelines. 1.5. Assists physicians/providers with various procedures. 1.6. Obtain pre-authorizations when necessary. 1.7. Convey instructions to patients/family members/caregivers. 1.8. Provides appropriate resource materials and referrals to patient/family as instructed by physician/provider. 1.9. Maintains patient records effectively. 1.10. Efficiently manages and routinely checks the EPIC in-basket. 1.11. Answers and returns calls promptly and provides pertinent information. 2. Performs administrative responsibilities as assigned. 2.1. Sends/receives patient medical records and abstracts new patient records into EPIC. 2.2. Obtains lab/radiology reports, hospital notes, and referral information as requested. 2.3. Completes forms/requisitions as needed. 2.4. Cross-trained on front desk duties, including but not limited to greeting patients/visitors, answering calls, scheduling appointments, and maintaining/scanning patient medical records. 3. Performs environmental responsibilities as assigned. 3.1. Sets up instruments and equipment according to department protocol. 3.2. Applies principles of infection control. 3.3. Cleans exam/procedures rooms, instruments, and equipment between patient visits to maintain infection control following established department policy. 3.4. Cleans the sterilizer (when applicable) according to the scheduled maintenance program and maintains appropriate documentation. 3.5. Orders, sorts, and stores supplies following established department policy. 4. Performs other related organizational duties as assigned. 4.1. Maintains patient autonomy, confidentiality, and dignity. 4.2. Ensures providers submit timely and accurate billing. 4.3. Maintains knowledge of practice policy, HIPAA, current coding, OSHA regulations and CLIA regulations (where applicable). 4.4. Provides prompt attention to urgent and non-urgent patient issues as evidenced by observation and feedback from staff. 4.5. Quickly shares problems related to patients and/or staff with the supervisor. 4.6. Work with the Care Coordinator for Patient Centered Medical Home initiatives. 4.7. Must meet one of the following requirements: graduation from an accredited medical assistant (MA) certificate program or Phlebotomy program, certification as a certified nursing assistant (CNA), emergency medical technician (EMT), or paramedic. Employees who join YNHHS through a merger or acquisition may satisfy educational requirements by demonstrating the appropriate combination of education, relevant experience, and current competencies. EXPERIENCE Minimum of one (1) year of experience as a medical assistant (MA), certified nursing assistant (CNA), emergency medical technician (EMT), ambulatory care associate (ACA), patient care associate/technician (PCA/PCT), paramedic, or phlebotomist required. New graduates from an accredited medical assistant certificate program, with less than one (1) year of experience, will be considered. LICENSURE Certification/registration (CCMA/CMA/AAMA/RMA) preferred. Certification/registration may be accepted in lieu of graduation from an accredited MA school. Certification in Basic Life Support (BLS) preferred. SPECIAL SKILLS Ability to follow directions effectively and consistently.
