Day (United States of America) Licensed Practical Nurse- Hospice (Field) The Field Licensed Practical Nurse provides direct patient care to hospice patients in a variety of settings and levels of care under the direct supervision of an RN. Days/Nights/Weekends/Holidays as assigned to provide 24 hours a day, 7 days a week care assigned to a variety of teams/duties. Professional working relationships are maintained with hospitals and nursing homes. ALFs, Physicians' offices and all internal and external customers Demonstrates nursing procedure in accordance with current hospital, hospice and team standards, policies and procedures. Patient needs are met appropriately and in a timely manner. Demonstrates a knowledge of best practice in end-of-life care and support, pain and symptom management. Documentation in the medical record reflects appropriate interventions are used for optimal and ongoing pain and symptom management Maintains communication with the community physicians/ hospice medical team Identifies educational needs and provides appropriate training to patient/family. Works as assigned to assist in providing direct patient care 24 hours a day, 7 days a week including holidays Meets or Exceeds productivity standards for assigned position Maintains all company owned equipment in good working order Participates in orientation, supervision and education of new employees as assigned.
