ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Educational Requirements: High School Diploma or GED Minimum Experience: A minimum of two (2) years clinical nursing experience with emphasis on care of adults in acute care medical and surgical areas is required for Hospice. A minimum of two (2) years clinical nursing experience with emphasis on care of adults in acute care medical and surgical areas is strongly preferred for Long Term Care. Demonstrates critical thinking skills Demonstrates sound clinical skills and judgment Basic computer skills necessary to operate computer systems used on unit Essential Tasks and Responsibilities: LTC LPN Assesses the patient’s physical and emotional needs and provides appropriate care and intervention. Monitors resident for changes in condition. Reports changes to Physician, Responsible Party, Nurse Supervisor, and other appropriate health professionals. Performs ongoing assessment and communicates assessment and plan of care through written documentation and verbal reports. Correctly administers and documents medication regime. Participates with instruction and education to resident and/or family to help insure safe and appropriate discharge. Contributes to resident safety by: utilizing principles of asepsis in all resident contact, providing a safe resident environment (i.e., Call light within reach, side rails up, etc.), maintaining properly functioning equipment and adhering to infection control policies and procedures. Assesses the status of all residents nursing care of the facility and consults with medical staff as needed. Identifies and implements infection control and safety practices Participates in the orientation of other employees. Organizes patient care and demonstrates ability to set priorities by addressing acute needs first and implementing Physicians’ orders within appropriate time frame. Assists in coordination of residents for admissions, transfers and discharges. Evaluates and monitors, utilization of equipment and supplies. Makes sure that adequate supplies stocked on the units and all residents charges are appropriately recorded. Regularly checks emergency equipment to see if they are in proper working order and that sufficient supplies are at hand. Helps keep the unit safe, clean and attractive by reporting to housekeeping any major problems and reporting to maintenance any safety issues. Area/Unit Specific Essential Tasks and Responsibilities: Hospice Plans nursing visits in accordance with agency policy and implements all aspects of the maintenance of current POT, including documentation in accordance with the orders as scheduled with no more than two variances, annually. Responds appropriately to emergency situations as indicated by the assessment findings and nature of the patient’s condition, in accordance with service policy and procedure. Provides hospice aide/homemaker duties and continuous care to appropriate patients when assigned. The LPN assists with the collection of data from a variety of sources to enhance clinical decisions about treatments and nursing interventions. The LPN provides feedback to the registered nurse in the development of nursing diagnoses and carries out the established plan of care and documents reassessments, interventions and patients’ responses to interventions. The LPN carries out therapeutic interventions established in the interdisciplinary plan of care. The LPN provides feedback to the registered nurse continuously in the evaluation of interventions and treatments and how they affect patient health status and expected outcomes. The LPN supports the evaluation of care activities, education and practice environments to ensure the provision of safe, quality care. The LPN participates in educational activities to improve clinical knowledge and enhance role performance.
