Primary Responsibilities: • Assist client with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and hygiene. • Assist with transfers, repositioning, wheelchair support, and mobility assistance using proper body mechanics and safety procedures. • Monitor client for changes in physical condition, behavior, speech, mobility, or cognitive status. • Support communication using verbal cues, communication boards, AAC tablet devices, or speech assistance techniques as needed. • Prepare meals and assist with feeding according to dietary restrictions and swallowing precautions. • Remind and/or assist client with medications as directed by care plan. • Maintain a safe and clean environment to reduce fall risks and promote comfort. • Assist with range-of-motion exercises and encourage participation in therapy-directed activities when instructed. • Provide companionship, emotional support, and respectful social interaction. • Document care provided, incidents, observations, and changes in condition accurately and promptly. • Communicate effectively with family members, nurses, therapists, and supervisors regarding client needs and concerns. • Follow infection control procedures, emergency protocols, and client confidentiality standards. Required Skills & Qualifications: • Experience caring for elderly, disabled, or stroke recovery clients preferred. • Knowledge of stroke-related conditions including mobility limitations, aphasia, paralysis, or swallowing difficulties preferred. • Ability to safely assist with transfers and mobility. • Compassionate, patient, dependable, and attentive demeanor. • Strong communication and observation skills. • CPR/First Aid certification preferred.
