The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required. MDS Coordinator RN The Sans Souci Rehabilitation & Nursing Center is looking for a talented and hard-working MDS Coordinator to join our ever-growing team. Description A nurse practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse.
