Job Details Our Cath Lab RN's: Manage overall care for patients undergoing cardiac diagnostic and interventional procedures. This includes sedation, preservation of the sterile field, ACLS management, documentation, etc. Partner with an interdisciplinary team including, but not limited to, attending physicians, registered nurses (RN), and ancillary staff to optimize patient outcomes and satisfaction. JOB SUMMARY: The Registered Nurse plans, organizes and directs home care services and is experienced in pediatric nursing from birth through late teens, with emphasis on community health education/experience. RESPONSIBILITIES: Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Working knowledge of all infection control procedures and safety precautions. Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process. Observe ethical and legal guidelines for safeguarding confidentiality of patients. Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. Participates in the identification of resident problems that require nursing intervention Evaluates residents for any decline of condition, signs of change in condition, weight loss, decline in ambulation, change in ADLs, development of skin breakdown, etc. Notifies appropriate staff per facility protocol of the need for evaluation and informs staff on any changes in assessment when appropriate Confer with residents and families on any issues relating to care and services Make referrals to other departments when appropriate Complete employee and resident incident reports and complete follow-up investigation forms Maintain accountability for controlled substances by reviewing inventory medications with another licensed nurse and notifying supervisors of any discrepancies Performs routine charting duties in accordance with established charting and documentation policies and procedures Supervise the CNAs, LPN’s and the unit clerk in regards to work procedures, expediting workflow, evaluations, counseling and discipline Performs and monitors the appropriate transfer of residents, including the use of transfer equipment. Performs other related job duties as required. Qualifications: Must have an active license as a RN Nurse (RN) in the State of New York. Graduate from an accredited School of Nursing Knowledge of practical nursing theory and practice. Ability to follow oral and written directions. Prepare and maintain records, charts, and reports. Ability to apply nursing techniques as directed by physicians or professional nurses. Ability to meet the general health requirements of this facility, including a medical and physical examination. Ability to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 3 feet and for a minimum distance of 3 feet.
