About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Tacoma-South Seattle area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. According to the educational plan determined by the professional provider, provides instruction to the patient and family/significant others. Provides instruction that matches learning needs and the nursing plan of care. Encourages patient compliance with therapeutic and medical regimens. Under the direction of the professional provider, provides the patient with preventive care information that matches his/her age, sex, and health status. Utilizes available health education resources. Reinforces the plan with patient to improve general health status. Accurately documents selected patient history observations and procedures. Adheres to KFHPW documentation standards. Provides each patient a safe, comfortable, and private environment. Provides a safe environment for each patient and adheres to KFHPW safety standards. Adheres to KFHPW infection control policies and procedures. Demonstrates responsibility and accountability for practice, for continued growth and self-evaluation.
