About Us: Our Pediatric Oncology Clinic is one of the leading pediatric cancer facilities on the West Coast, offers specialized services for a wide range of cancer diagnoses. As the sole provider of pediatric oncology services between Los Angeles and the Bay Area, we see approximately 40 outpatient visits daily and bring a wealth of experience and expertise to each patient we treat. Our Cancer and Blood Disorders Center consistently ranks in the top 5 to 8 percent globally in patient enrollments for the Children’s Oncology Group (COG), the world’s largest organization focused on childhood and adolescent cancer research. Our dedicated team conducts a wide range of treatments, from chemotherapy and immune suppressant medication to laser therapy, blood transfusions, bone marrow aspiration and biopsies, lumbar punctures, chemotherapy administration in the cerebrospinal fluid, and radiation. Our outpatient unit houses treatment rooms, an infusion center, and playroom, all designed for family centered care. Among our patients, approximately 45% are diagnosed with leukemia and lymphoma, while the rest include solid tumor, neuro-oncology, histiocytosis, and vascular malformation cases. With an impressive 85% survival rate, we offer an exceptional Childhood Cancer Survivorship Program for patients completing treatment. This program focuses on easing the transition back to life before cancer and addressing potential long-term side effects of treatment, both physical and emotional. Job Summary: The Registered Nurse (RN) is accountable for providing competent nursing care. Overview Community Health Partners is seeking a full-time Advanced Practice Provider (APP) to join the Neurocritical Care team. In this role, the APP will provide comprehensive care for patients with critical neurologic illness, including acute stroke, intracranial hemorrhage, status epilepticus, and patients recovering from neurosurgical and neurointerventional procedures, in collaboration with the interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, prescribing appropriate therapies, and performing critical care bedside procedures. The APP will support patients throughout the critical care continuum and ensure high standards of safety and quality. This position follows a 7-on, 7-off schedule with day and night shifts. Night shift responsibilities are assigned based on experience and training. Our Neurosciences department now includes the Bob Smittcamp Family Neurosciences Institute, a five-story, 60,000-square-foot facility on the Community Regional Medical Center campus that opened in May 2026. Funded by $30 million in community donations, the Institute brings together more than 60 physicians and 30 advanced practice providers across stroke and neurovascular care, epilepsy, neuro-oncology, traumatic brain and spine injury, movement disorders, and neuromuscular disease. Provides clinical data and observations to contribute to the nursing plan of care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient report and assignments with Nursing Assistants upon the beginning of assigned shift • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to observe the resident’s physical and emotional status • Collects clinical data and reports significant clinical findings according to policy • Performs manual and technical skills according to the scope of practice under the direction of a Registered Nurse • Records care information accurately, timely and concisely. Completes all required documentation including resident observations, interventions, and patient response(s) in the medical record in accordance with policy • Assists in developing, reviewing, revising, and updating resident Plans of Care as indicated. Contributes to the evaluation of the patient’s progress towards specific goals and the adjustment of the nursing plan of care as necessary • Receives and transcribes orders accurately from attending/alternate physician. • Assists in making rounds with the attending physician, as necessary. • Prepares/administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates response and report adverse reactions to medication or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Notifies the attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within established policies. • Notifies the attending/alternate physician of resident incidents/unusual occurrences. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Assists with a gathering of information relative to the event. • Initiates emergency procedures (including basic CPR) and provide emergency patient treatments • Participates in the facility’s infection control program for monitoring communicable and/or infectious disease among residents and staff. • Participates in procedures for reporting hazardous conditions or equipment. • Assists in orientation of newly assigned personnel as requested. • Assumes Charge responsibility following orientation period as requested. • Performs other position-related duties as assigned. • Supervise CNAs SUPERVISION • Make resident rounds to ensure appropriate care is being rendered, identifying, and making corrections as necessary • Meet with nursing personnel to assist in identifying and correcting problem areas and/or the improvement of resident care • Assist the Charge Nurse in revision and distribution of nursing assignments as needed • Assist in the supervision and direction of nursing personnel • Participate in disciplinary action and evaluation of nursing personnel CONSUMER SERVICE • Presents a professional image to consumers through dress, behavior, and speech. • Treats residents/family members with dignity and respect • Responds to Resident and family concern(s) and assists in resolving grievances. Assist in correcting findings and deficiencies from the Department of Health Services, Consultants, and the Quality Assurance Team as directed by the Director of Nursing. Actively participates in Quality Assurance Performance Improvement teams and activities • Ensures the protection of health information as required by the Health Insurance Portability and Accountability Act (HIPAA) • Participates in the facility’s Infection Control Program for monitoring communicable and/or infectious disease among residents and staff • Participate in procedures for reporting hazardous conditions or equipment. • Orient new staff members upon request • Participate in the facility's quality assurance (QA) program as requested or needed.
