One of the most important aspects of being a Registered Nurse Case Manager is building trusting relationships with patients and their families. This trust allows us to serve as the central point of communication within our interdisciplinary team of holistic care providers. Hospice nurses play a vital role in helping patients spend their final days in a way that truly reflects who each individual is and what matters to them most.
