What We’re Looking For: We’re looking for a dynamic Director of Nursing to lead our clinical team and provide our residents with an exceptional care experience. We need you to plan, organize, develop and direct the overall operation of your Nursing Service Department in accordance with current federal, state, and local standards, guidelines and regulations. Salary is based on experience. What You Will Do: Interview, hire, and train employees; plan, assign, and direct work; appraise performance; reward and discipline employees; address complaints and resolve problems Coordinate care and services to improve organization and implementation of plans of care, and quality of life and for all residents. Ensure proper assessment of all prospective residents prior to admission including visiting client at their home or in the hospital. Develop staffing plans that assure sufficient staffing to meet direct care needs, recommend numbers and types of nursing personnel necessary to provide care and maintain compliance Ensure 24hour availability as nursing administration resource by providing on-call and telephone access or designating an alternative, qualified nursing staff member to do so. Audit clinical records for accuracy and completeness of comprehensive resident assessments, effective documentation reflecting resident responses to interventions and consistent implementation of plans of care by all staff and professionals, on all shifts. Conduct quality assessment and assurance activities, including regulatory compliance rounds, in all departments to monitor performance and to continuously improve quality Lead and participate in interdisciplinary conferences as needed to develop adequate plans of care. Establish and maintain established policies, procedures, CQI programs and safety, environmental and infection control standards. Provide direct administrative and technical supervision of nursing, admissions and continuing care functions. Establish performance goals. Provide leadership in planning, organization, evaluation and fiscal management Collaborate with executive management team on Goals of our home Collaborate with interdisciplinary team members to develop and promote programs to enhance the resident’s quality of life and healthcare Ensure compliance with applicable local, state and federal regulations and the policies and procedures. Assist in the preparation of departmental budgets and allocation of funds based upon a review of department estimates and recommendations. Completes all required documentation including resident observations, interventions, and patient response(s) in the medical record in accordance with policy • Assists in developing, reviewing, revising, and updating resident Plans of Care as indicated. Contributes to the evaluation of the patient’s progress towards specific goals and the adjustment of the nursing plan of care as necessary • Receives and transcribes orders accurately from attending/alternate physician. • Assists in making rounds with the attending physician, as necessary. • Prepares/administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates response and report adverse reactions to medication or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Notifies the attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within established policies. • Notifies the attending/alternate physician of resident incidents/unusual occurrences. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Job Summary: The ICD Nurse Specialist role at Kaiser Permanente Southern California Region is a nursing role with the critical element being expert clinical practice. The ICD Nurse Specialist functions under the medical direction of the Electrophysiologist and/or Cardiologist evaluating, educating, and performing device programming, for patients with implantable cardiac devices (ICD and pacemaker). Essential Responsibilities: To ensure that nursing care needs of the cardiac device patient population are met, provides 24 hour clinical leadership by developing standards of practice, policies and procedures for cardiac device patient population that are consistent with professional practice standards. Implementing methods and processes in specialty area to support change in nursing practice, care programs and clinical innovation. Achieving improved patient/family outcomes. Independently assess cardiac device transmissions, including electrograms, from patients using knowledge and experience in device programming. Functions independently in the assessment and interpretation of cardiac device data from patients in outpatient clinic, inpatient hospital, and by remote interrogation. Provides medical and/or technical programming recommendations and advice. Performs interrogations and reprogramming as needed, on all types of implantable cardiac devices. Evaluates clinical practice in cardiac device patients to ensure community/professional standards are met. Effectively identifies, evaluates and addresses disease prevention and health promotion issue of the cardiac population with implanted devices, while administering quality patient care. Provides educational assistance to Cardiology and EP Fellows and Family Practice/ Internal Medicine Residents on cardiac devices. Coordinates ICD patient/family support group. Schedules, coordinates, and performs interrogation. Interprets and evaluates therapeutic and diagnostic tests on cardiac device patients working with the Electrophysiologists and/or Cardiologists. Explains necessity, preparation, nature, and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient. Records physical findings and formulates plan, based on patients condition. Consults and collaborates with physician and other healthcare providers as needed regarding patient care management issues and in the development of comprehensive patient care plan. Submits healthcare plans and goals for periodic review by physicians. Maintains accurate and confidential medical records. Documents all procedures, evaluations, treatments, outcomes, education, and referrals, consistent with NCQA (National Committee for Quality Assurance), JCAHO (Joint Commission on Accreditation of HealthCare Organizations) and state regulatory standards. Analyzes new knowledge gained from conferences, workshops, professional literature, or hands on training and assimilates this knowledge into clinical practice. Assists in interviewing staff and provides input into evaluations regarding clinical competence and provides coaching and counseling. Performs other duties as assigned.

Understands actions, side-effects, contra-indications of drugs. Follows five “rights” of medication administration. Adheres to medication policies, practices and standards. Administers medication, intravenous therapy and fluid management only on physician request and guidance. Obtains necessary training prior to initial equipment use. Assures equipment is in operating order prior to use. Differentiates between patient complications and equipment malfunction. A nursing assistant is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS • Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. • A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. • Must possess a sincere desire to serve the ill, aged and infirm. • Ability to read, write and speak the English language. • Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. • Willingness to work harmoniously with other personnel. • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. • Must be in good general health and demonstrate emotional stability. • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company Standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.
