Position Summary Seeking an APRN for a flexible outpatient role supporting services through extended service rotations, providing several consecutive months on each team to promote deeper clinical immersion and continuity of care. Serve as a primary clinical provider for infants, children, and adolescents in these outpatient clinics: Gastroenterology, Allergy, Pulmonary, Infectious Disease, and primary care. Initial coverage will be in Infusion. This role works collaboratively with physicians, nursing staff, and interdisciplinary teams to provide direct patient care, care coordination, triage, and family education for children with complex and chronic conditions. The APRN supports both scheduled clinic visits and ambulatory treatments, with rotating schedules to meet program and patient needs. Key Responsibilities · A mbulatory outreach clinics: Provide direct patient care for new and returning pediatric patients- 6 sessions per week. · Infusion/day treatment- Deliver clinical coverage in an ambulatory clinic serving over 8,000 patients annually, including treatments such as biologic infusions, blood products, food challenges, and day treatment for newly diagnosed diabetes, migraine management, sickle cell fever and pain, and central line complications · Independently managing infusion and acute reactions · Serve as a primary provider, facilitating patient care including medical histories, physical examinations, diagnostic evaluation, and development and implementation of evidence-based treatment plans for both new and return patients · Collaborate closely with physicians, nursing teams, and interdisciplinary colleagues to promote optimal clinical outcomes and patient safety. · Perform patient triage, clinical care coordination, and on-site coverage for urgent patient needs. · Serve as a primary communicator for patient and family questions, including phone triage for outpatient concerns. · Assist with medication refills, prior authorizations, and coordination of long-term treatment plans. · Orientation includes a structured, individualized onboarding program with additional training when starting in each new department, tailored to each APP’s experience level, clinical background, and learning needs. As a Licensed Practical Nurse, your main role will be to deliver nursing care to our residents within a warm, comfortable, and home-like environment. Responsibilities Accurately assesses, communicates, and documents residents’ status. Observes and reports any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations. Directs and supervises the daily work assignments of the Resident Care Assistants. Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job. Coordinates care needs with community providers via an effective case management process. Effectively balances service demands with supporting resident independence. Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations. Responds to emergencies and personal emergency response system and knows when to call for backup.
