Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line.
