The work schedule may vary within the site operating hours of 8:00am-6:00pm, Monday-Friday. A minimum of 1 year of proven clinical leadership experience is required. What You’ll Do Collaborates with Site Medical Leader to provide leadership to health center staff Assists in the recruitment and selection of staff members, including interviewing candidates and training new staff members Assesses, manages, and evaluates the performance and development of staff members Manages and monitors clinic staff schedules Oversees the nursing care of all patients Provides direct patient care < 30% of the time. Identifies emergency situations, and provides care within legal scope of practice Participates in appropriate client and Premise Health committees as requested Directs preparation and maintenance of patients' clinical records Provides direction to ensure environment is clean and safe, that infection control procedures are followed, and patient privacy is upheld Oversees ordering and maintenance of supplies, drugs, solutions, and equipment, and maintains records on narcotics, if applicable Participates in Quality Improvement, Risk Management activities and completes Premise Health occurrence reports as needed. Description P osition Title: Licensed Practical Nurse-Greenwich Village Clinic Nursing-Mount Sinai Hospital-Full Time-Days Position Summary: The practice of nursing as a Licensed Practical Nurse is defined as performing tasks & responsibilities within the scope of supportive & restorative care; medication administration and specialized tasks under the direction of a Registered Professional Nurse of licensed or otherwise legally authorized physician or dentist. Responsibilities A. Clinical/Technical/Service Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse Performs job duties under supervision of a Registered Professional Nurse. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. Administer IV fluids, provided completion of annual competency IV therapy training. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. Makes recommendations in the application of the nursing process. Participates in planning, and administers direct patient care, including activities of daily living (ADL). Performs point of care testing including blood glucose. Performs nursing procedures as defined in the nursing procedure manual. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. Makes, reports and records observations on assigned patients within the EMR. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. Participates in health teaching to patients and families following the RNs development of the teaching plan. Establishes an environment that promotes optimum physical and emotional comfort. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Qualifications Education and Experience requirements: Completion of educational program for licensure as a LPN. Successful completion of required New York State Department of Health LPN IV Therapy Initial Program for the demonstration of competence in the performance of intravenous therapy/blood administration procedures as part of Centralized Orientation. Successful completion of annual unit-based review & update program in procedures associated with intravenous therapy. Fingering: Occasionally occurs while documenting patients’ medical charts. Handling: Constantly handling patients' personal items, blood pressure gauge, thermometer, and eating utensils. Torquing: Not required. Controls & Equipment: Blood pressure gauge, thermometer, etc. Other Physical Considerations: Twisting: Occasionally may occur while transferring patient. Bending: Occasionally occurs while taking vital signs, assisting patient in bathing, providing peri-care, etc. Squatting: Occasionally may occur while bathing patient; also required when putting on shoes and socks of patients, and also when making eye-to-eye contact while talking if patient is in a wheelchair. Kneeling: Occasionally may occur while bathing patient. Crouching: Required when putting on shoes and socks of patients, and also when making eye-to-eye contact while talking if patient is in a wheelchair. Climbing: Not required. Balancing: Not required. EQUIPMENT USED Blood pressure gauge, thermometer, etc. SUMMARY OF OCCUPATIONAL EXPOSURES Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious disease, air contaminants and hazardous chemicals. May be subject to hostile and emotionally upset patients, family members, associates and visitors. OTHER CONSIDERATIONS AND REQUIREMENTS In this position, the associate is required to be on his or her feet continually throughout the day. In a full-time nonrestrictive basis, the associate must be able to lift at least 40 pounds of weight, which is required when transferring a patient from a bed to a wheelchair. A transfer belt is necessary and required when transferring any patient. This is for the safety of not only the patient, but also the aide. Other nurses are available to assist with heavier patients as needed.
