JOB DESCRIPTION SUMMARY As an Admissions Nurse, you are responsible as a member of the Interdisciplinary Team for the provision of skilled nursing assessment, planning and care on an on-call basis in order to maximize the comfort and health of patients and families consistent with Hospice policies & procedures. You will collaborate with the patient, caregivers, clinical manager, case managers, and physician(s) to complete the patient care planning at the onset of care. You will provide skilled interventions and instructions to the patient and caregivers to provide optimum symptom control and patient comfort. You will function as a liaison between disciplines during the admission process. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of the nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Ohio Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of GA 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 5. Job Description Coordinates care of patients and families within accepted practices, policies, and philosophy of Children’s Healthcare of Atlanta. Coordinates clinical management, referral, evaluation, communication, regulatory, and research aspects of clinical trials and data management for potential candidates, patients, and families and provides appropriate follow-up care. Experience 4 years of experience in nursing 2 years of experience in specialty nursing in appropriate area Preferred Qualifications Master’s degree in Nursing, Healthcare, Business, or related field Previous experience with Dialysis, Nephrology ICU, Transplant experience helpful Education Bachelor of Science in nursing Certification Summary Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact Basic Life Support (BLS) certification from the American Heart Association within 30 days of employment Knowledge, Skills, and Abilities Demonstrated computer skills in database management Must be able to successfully pass the Basic Windows Skill Assessment at 80% or higher rating within 30 days of employment Job Responsibilities Coordinates evaluation and consultation process for patients in inpatient and outpatient settings and ensures appropriate documentation and follow-up of tests and coordination of care. Ensures appropriate outpatient care and follow-up by receiving and returning outpatient phone calls in a timely fashion and providing documents and medication refills as needed. Develops, maintains, and implements protocols, data collection tools, and communication mediums to ensure research patients, gathering and recording research/quality improvement data, and coordinating follow-up appointments. Provides input and assists in developing policies and procedures and coordinating regulatory accreditation and audit process to support continuous assessment of patient care and develop methods for improvement. Establishes relationships with departments and managers to ensure program activities comply with organizational guidelines and regulatory standards. Develops instruction and information programs for staff to educate on program criteria, current developments, project status, and how program activities impact the organization. Performs other activities as necessary to ensure and provide a safe and therapeutic continuity of patient care. Demonstrates competencies that incorporate age-specific guidelines, including N = Neonate (less than 30 days), I = Infant (30 days to 1 year), EC = Early Childhood (1-5 years), LC = Late Childhood (5-13 years), A = Adolescent (13-17 years), AD = Adult, and ALL = all ages.
