Salary Range : $97.09 - $129.43 Summary of Duties : The Staff Nurse III is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame and provides fiscally responsible care to these patients via therapeutic use of self, the nursing process, the environment/instrumentation and other health care team members. He/she demonstrates excellence in nursing practice, thoroughness, creativity, leadership, education and participation in unit projects. Education Requirements : Graduate of an accredited school of nursing with a Bachelor's degree in nursing is required or Master's Degree in nursing - PLUS experience as outlined below. Licensure/Certification Requirements : Current California RN license required. AHA BLS and PALS required. A licensed professional nurse under the supervision of a Registered Nurse who provides nursing care and services to residents in a long-term care setting. Provides clinical data and observations to contribute to the nursing plan of care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient report and assignments with Nursing Assistants upon the beginning of assigned shift • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to observe the resident’s physical and emotional status • Collects clinical data and reports significant clinical findings according to policy • Performs manual and technical skills according to the scope of practice under the direction of a Registered Nurse • Records care information accurately, timely and concisely. Completes all required documentation including resident observations, interventions, and patient response(s) in the medical record in accordance with policy • Assists in developing, reviewing, revising, and updating resident Plans of Care as indicated. Contributes to the evaluation of the patient’s progress towards specific goals and the adjustment of the nursing plan of care as necessary • Receives and transcribes orders accurately from attending/alternate physician. • Assists in making rounds with the attending physician, as necessary. • Prepares/administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates response and report adverse reactions to medication or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Notifies the attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within established policies. • Notifies the attending/alternate physician of resident incidents/unusual occurrences. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Salary Range: $92.48 - $123.29 + applicable differentials Summary of Duties : As part of a multidisciplinary team, provide care to cardiac, vascular, interventional radiology and neuro patients in the Cath Lab. Hemodynamic monitoring, circulating, and sedation administration during invasive procedures. Education Graduate of an accredited school of nursing required. BSN preferred. NIH Stoke certification required, can be obtained after 3 months of hire. License(s)/certification(s) Current CA RN license, AHA ACLS, BLS required. Preferred CCRN, CEN or RCIS certification. Proficient in IV starts and EKG interpretation. Experience: Minimum 2 years acute care hospital experience and 1 year Cath Lab experience required . Critical Care experience required (CCU, ED). Moderate Sedation experience required . Special skills/abilities Strong Cardiac, Structural Heart, Neuro IR background. Ability to stand long periods with Lead aprons. Sedation competency. Ability to hold femoral artery pressure for 30 mins. Ability to support face paced interventional lab. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. Qualifications QUALIFICATIONS: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred. • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
