Job Description: Maya Home Care is seeking a dynamic and driven Home Care Director of Nursing to join our team. The Director of Nursing (DON) is responsible for the overall clinical management, supervision, and coordination of nursing services provided by the agency. Prepare for and participate in state surveys, accreditation reviews, and audits. Implement corrective action plans as needed following surveys or quality reviews. Clinical Oversight Review and approve patient assessments, plans of care, and clinical documentation as required. Monitor the appropriateness and quality of patient care services. Ensure timely physician orders, recertifications, and clinical documentation. Evaluate patient outcomes and implement interventions to improve quality of care. Participate in case conferences and interdisciplinary care planning. Staff Supervision Supervise and evaluate nursing personnel. Assist with recruitment, interviewing, hiring, orientation, and retention of clinical staff. Conduct performance evaluations and competency assessments. Ensure staff maintain required licensure, certifications, and continuing education. Provide ongoing clinical education and professional development opportunities. Quality Assurance and Performance Improvement Direct the agency's Quality Assurance and Performance Improvement (QAPI) program. Monitor clinical indicators, patient satisfaction, incident reports, infection control data, and quality metrics. Analyze trends and implement performance improvement initiatives. Position Summary (CMA) Under the direction of the physician, Charge RN or Manager performs direct clinical activities in a courteous, caring and respectful manner. Responsible for nursing assessment, writing of diagnoses, developing and intervening in the persons receiving services health care plans to ensure quality care in the most efficient manner. Advocates for health and accesses hospital and specialty clinic services. Provides supervision to the Licensed Practical Nurse(s) on shift. POSITION DUTIES: Assesses each person receiving services to determine and identify needs, issues, resources, and health care outcomes. Identifies individual’s development strengths and health care needs. Obtains emergency medical care when necessary. Develops and updates health care plan to address assessed needs. Provides direct medical/nursing services to assigned individuals as presented. Oversees the implementation of the health care plan by training persons receiving services, family, and/or caregivers. Trains and oversees staff in carrying out treatment plans relating to health needs on an ongoing basis. Evaluates effectiveness of plan in meeting outcomes. Communicates outcomes to patients, families, caregivers, or service providers. Refers to other health providers, as needed. Schedules follow-up medical appointments according to medical recommendations. Ensures proper documentation of persons receiving services care and progress. Develops and updates patient’s profile with the Residential Coordinator or other service providers for use at clinic appointments and hospitals, to include presenting problems and current medication list. Conducts on-going reviews of medication administration documentation and monthly reviews of medexes. Conducts regularly scheduled observations and pouring (procedures) of medication administration procedures performed by medication certified personnel. In conjunction with the Residential Coordinator, supervises AMAP staff and works with the Nursing Educator to ensure all AMAP staff are initially certified and annually re-certified. Maintains all medical standards as set forth in agency policy and OPWDD regulations. Participates in meetings as indicated including but not limited to IDT, Day Program, Human Rights, Departmental, Borough and specially arranged meetings. Performs all other duties as assigned or indicated including but not limited to: completing surrogate packets, writing incident reports, assisting in developing policies, procedures and forms; and establishing working systems.
