Department Medical Management Services Job Summary Manages the health status of the member population under the direction of the plan Medical Director. The Clinical Care Facilitator will manage the medical care throughout the continuum of care. The only Level III intensive care nursery in the area, our NICU is equipped with the latest technology and multi-disciplinary specialists to provide advanced life support and around-the-clock specialty care and monitoring.
