JOB DESCRIPTION: The Certified Nursing Assistant works directly under the supervision of the Registered Nurse or Licensed Practical Nurse (LPN) and performs duties in a relation to the personal needs of the resident, care of supplies and equipment, and maintaining the cleanliness, comfort and safety of the resident in his/her environment. REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS) Ability to comprehend and follow verbal and written instructions. Ability to write legibly. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Residents Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Engages in cluster feeding while seated. Monitors daily intake and keeps nurses informed. Records food intake on Nutrition Work Sheet. Making Beds Uses recommended amount of linen and diapers Disposes of linen in proper manner. Properly utilizes clean and dirty linen carts. Secures call bells to bed. Beds are made in proper manner. Knowledgeable in procedure for linen precaution. Safety of Residents: Restraints are to be properly applied and released and removed every two (2) hours for at least 15 minutes. Make sure, if restraint is used, that it was indicated on assignment and the proper type is applied. Restraints are checked at the beginning and end of each shift. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. Rehabilitation: Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Cooperates with all rehab disciplines. Keeps accountability sheets up to date with signatures as required. Has knowledge of proper procedure. Records results. Informs nurse of results. Assignments: Receives daily report from nurse. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident. Routinely checks and keeps in order all residents' belongings: i.e. clothing, glasses, hearing aid, prosthesis aid, adaptive devices, etc. Ensures that residents on turning schedule is positioned every two hours or as ordered, and documented. Ensures that residents on toileting schedule are assisted as needed, and documented. Resident is kept clean and dry. Gives assistance to co-workers when needed. Uses good handwashing techniques. Provides direct hygiene and health instruction to staff, students receiving services and families as required. Handles control of communicable diseases and infections through identification and assessment. Reports to consulting physician and Infection Control Nurse. Implements appropriate protective and preventive measures. Provides staff training in regard to infection control regularly and as needed. Provides first aid to students in need of immediate care, with referral to registered nurse, private physician or outside emergency services as necessary. Monitors required staff health records and provides tests in accordance with regulatory agency requirements. Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data of students via progress notes. Administers medications as prescribed. Supervises secured storage of medications. Refers all consults and diagnostic reports to registered nurse for review. Transports students to appointments as necessary. Maintains Nursing Office. Orders supplies and equipment as required. Visits students at home as requested by Director or designee. Keeps abreast of changes in Nursing Services field. Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. Complete accident/incident reports, as necessary. Provide information to the Billing Office as necessary to insure accurate resident billing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
