Utilizing the nursing process, the Nurse Navigator provides patient education and access to relevant internal and external resources. Primary objectives are to facilitate informed decision making and timely access to high quality health and psychosocial care through all phases of the care continuum. Routine collaboration with internal departments and external referral resources are anticipated, including scheduling, financial counseling, lab, pharmacy, social work, nursing, surgical, radiation and imaging, dietary, and others. Regular and frequent interface with practice-based leadership teams, physicians, and clinical staff is required to promote and engage the patient navigation process. Performs coordination of care during the full continuum of the patient experience Identifies potential and real gaps in care delivery; seeks out opportunities for avoidance and/or closure Prepares for, organizes and leads the Patient Care Conference (huddle) at regular intervals; communicates interventional outcomes to full care team in a timely manner Performs assessment of patient risk stratification Performs basic psychosocial assessments and provides emotional support; delegates and/or make referrals for additional screening and support as needed Develops appropriate nursing diagnoses and corresponding action plans as needed Supports and/or provides education, adherence and side effect monitoring Holds or participates in survivorship and advance care planning visits; makes referrals as needed Participates in research, quality, and performance improvement, including reporting on navigation metrics Supports patient advocacy and empowerment Knowledgeable of practice operations guidelines, practice organization and healthcare economics SALARY LEVEL: $85,000-125,000 annually Qualifications Education/Training - Graduate from an accredited program for nursing education required (BSN preferred) Current licensure as a registered nurse in state of practice, Nurse Navigator Certification preferred Current BLCS or ACLS certification required Business Experience - Minimum five (5) years of nursing experience required Basic knowledge of local community resources Colorectal experience preferred Working Conditions: Traditional clinic environment The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. This includes establishing an intentional therapeutic relationship between a registered nurse and a patient and family. As a leader and the integrator of care, the professional nurse has the responsibility, authority, and accountability for planning, coordinating and evaluating the patient’s care needs. Works alongside the care team screening clients and/or population for healthcare needs and developing a client-focused case management plan. Principle Responsibilities Comprehensive assessment of patient and family needs; identification of outcomes based on needs assessment; establishing the plan of care; supervising the progress; and modification of the plan of care as needed. Maintains education and competencies to remain proficient to provide high quality patient care practice and skills. Incorporates evidence-based nursing practice taking into consideration current statutes, rules and regulations when developing the plan of care. Establishes, in the collaboration with the family and patient, realistic and measurable patient expected outcomes based on nursing diagnoses, patients present and potential capabilities, goals, available resources and plan for continuity of care. Identifies outcomes that are age appropriate, culturally appropriate, realistic and attainable. Coordinates, collaborates, and delegates appropriately to ensure care is provided in a safe, effective fashion. Fulfills all organizational requirements. Supplemental roles: Charge Nurse (only when acting in this role). ◦ Demonstrates ability to coordinate, delegate, and direct unit operation, collaborates effectively with unit staff, leadership and other disciplines. Preceptor (only when acting in this role). ◦ Demonstrates ability to identify the orientee's learning needs, plans appropriate learning experiences and demonstrates ability to validate clinical competence of orientee. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
