Ensures patient is in proper position and alignment. Assist RN and/or LVN/LPN with complicated patient treatment procedures. Serves and removes patient meal tray in timely manner. Assists with feeding and/or preparing items such as opening mild container, cutting food for patient. Assists patients with range of motion exercises, and movement to wheelchair or activity areas. Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards. Documentation via electronic health record. Attend interdisciplinary meetings as needed to provide input into selected patient’s plans of care. Documents restorative services provided following facility policies and procedures. Knowledge/Skills/Abilities/Expectations Excellent oral and written communication and interpersonal skills. Ability to react decisively and quickly in emergency situations. Ability to communicate effectively with patients and their family members, and facility staff. Basic computer knowledge. Approximate percent of time required to travel: 0-10% Must read, write and speak fluent English. Must have good and regular attendance. Performs other related duties as assigned. Qualifications Education High school diploma or equivalent preferred. Licenses/Certification Valid certification as a CNA in the state of employment. CPR certification required. Experience Six months experience in a long-term care environment preferred.
