We manage a wide range of diagnoses, including respiratory failure & complex respiratory disease progression, septic shock, heart failure, diabetic ketoacidosis, metabolic crises, post-lung transplant care, and patients with chronic medical complexity. CPR certification is recommended (required for direct patient care in Georgia), along with prior experience in hospice, home health, medical-surgical, or population-specific nursing such as geriatrics or pediatrics. Must demonstrate a willingness to participate in an on-call rotation and maintain reliable transportation, valid driver’s license, and state-required liability auto insurance. Strong communication and organizational skills are essential, along with proficiency in Microsoft Office Suite and EMR systems. The ability to collaborate effectively within an interdisciplinary team is required. This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching.
