Overview The Nurse Practitioner or Physician Assistant will be responsible for providing premium primary medical care services to a wide variety of patients. The Nurse Practitioner or Physician Assistant will be expected to systematically assess patients’ health status, arrive at appropriate diagnosis(es), and implement care plan(s) or effective clinical outcomes while meeting or exceeding quality metrics. The Nurse Practitioner or Physician Assistant works collaboratively with our physicians and patient centered care team to provide medical care and treatment in the most cost effective and appropriate way. Responsibilities The Nurse Practitioner or Physician Assistant will be responsible for providing premium primary medical care services to a wide variety of patients. Obtains patient history and performs assessment via observation, interview, and examination. Orders, performs and interprets diagnostic procedures, when appropriate by following appropriate medical practice standards. Participates with the physician(s) in the evaluation and management of patient care, as a member of the patients' care team. Performs procedures, suturing, wound care, and infection management, while thoroughly documenting such in the patient's medical record. Thoroughly and concisely documents in the patient's medical record all patient contact encounters, progress notes, orders, prescriptions, test results, and patient instructions according to standard medical practice guidelines and to support appropriate billing practices, in a timely manner. Educates and encourages patients to become active participants in their own healthcare-related behavior and provides patient education materials related to diagnosis, treatment plans, and need for follow-up care, for use in encouraging wellness Provides support for vacation coverage by responding to patient calls, patient portal messages, prescription requests, and follow up to test results. Able to provide telephone call coverage, as requested. Participates and contributes to quality reporting metrics, as indicated and appropriate. Manages patient care through appropriate use of available resources, including available technology and technologic platforms including but not limited to the electronic medical records, patient portal, telephone system, interfaced testing results, or other systems/ methods. Position Summary The Advanced Practice Registered Nurse collaborates with physicians and interdisciplinary teams to provide comprehensive palliative and hospice care in various settings. The APRN delivers expert clinical evaluation, treatment, and management of patients with serious or terminal illnesses. Responsibilities include assessing patient needs, facilitating goals of care discussions, prescribing treatments, and supporting transitions of care. The APRN plays an integral role in educating staff, families, and the broader medical community about palliative and hospice care. PRN hours M-F days to include some weekend availability if needed. Essential Functions Provide medical expertise in pain and symptom management, including terminal prognosis evaluation. Act as attending provider in hospice contract beds in collaboration with the hospice physician. Coordinate admissions and discharges for hospice contract beds, including review of terminal diagnoses. Assess patient conditions, develop care plans, implement interventions, and evaluate outcomes. Conduct patient and family assessments, ensuring readiness for transition to hospice or home settings. Perform physical exams, diagnostic testing, treatment, follow-up, and consultations. Prescribe and administer medications and other therapeutic interventions. Collaborate and consult with hospice/palliative physicians, attending physicians, and interdisciplinary team. Support patients and families in clarifying care goals and end-of-life decisions. Maintain accurate and complete medical records to ensure compliance and reimbursement. Ensure informed consent documentation, including CPR and DNR discussions. Participate in interdisciplinary team meetings and on-call rotations. Educate internal staff and community members on palliative and hospice care. Coordinate inpatient APRN assignments and support departmental functions . Qualifications Knowledge and Critical Skills Expertise in patient charting, history taking, and documentation practices. Knowledge of health education methods, clinical operations, and medical procedures. Proficient in diagnosis, treatment planning, and symptom management. Excellent interpersonal, verbal, and written communication skills. Ability to organize, analyze, and prioritize complex tasks and data. Comfortable with public speaking, leadership, and team collaboration. Experience Minimum two (2) years of nursing experience. Experience in hospice or palliative care preferred. License / Certification Current Advanced Practice Nurse or Physician Assistant license in the State of Florida. Valid Florida driver’s license with acceptable driving record and reliable transportation. Education Master’s degree from an accredited Advanced Practice Nursing program. Overview The Registered Professional Nurse is responsible for identifying the patient's alterations in health, assessing the patient's human response to actual and potential health problems, implementing nursing interventions based on theoretical concepts from nursing and other arts and sciences and evaluating the effectiveness of nursing interventions to achieve optimal patient outcomes. Overview The Lead Licensed Practical Nurse is responsible for nursing staff coordination within the practice. The Lead Licensed Practical Nurse will participate in the care of patients in a physician office setting by measuring and recording vital signs, placing patients in exam rooms, preparing patients for exams/procedures, assisting the health care provider with exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. Overview For Inpatient assignment, assists the attending physician in the care and management of patients on Inpatient units within the hospital. This includes making patient rounds, contributing to the decisions for daily care, writing orders, interacting with consultants, and facilitating and coordinating the scheduling of therapeutic and diagnostic tests and procedures. Will make frequent bedside visits to assess the status of patients and direct care accordingly. May also see patients in the clinic environment. For Emergency Dept. assignment assists the Emergency Department physicians in the comprehensive medical care of the patient, in the emergency department and the chest pain observation unit and in any role the supervisory physician deems appropriate and consistent with applicable law, hospital regulation and licensure and clinical privileges. The incumbent will consult with the supervisory physician to determine the most appropriate care for patients. For Clinic assignment, under supervision of an attending physician this position will provide diagnostic and therapeutic health care coverage for patients which will include but not be limited to taking medical histories, performing physical examinations, coordinating lab tests, diagnostic studies, prescribing medications, and providing assistance to the patient regarding a plan of treatment and education. Other duties may include sharing call hours, phone coverage, and seeing admitted patients within the hospital. For ICU assignment assists the attending ICU physician in the care and management of patients. This includes making patient rounds, contributing to the decisions for daily care, writing orders, interacting with consultants, and facilitating and coordinating the scheduling of therapeutic and diagnostic tests and procedures.
