Position Summary The weekend hospice registered nurse works Friday 8am-4:30am, Saturday 7am-7:30pm, and Sunday 7am-7:30pm, and is included in the on call schedule on a rotating basis. The weekend hospice RN is responsible for triaging, providing skilled nursing care, assessments, symptom management, education, and support to hospice patients and their families. The RN works collaboratively with the hospice team to ensure continuity of care and implementation of the hospice plan of care. Director of Care – Skilled Nursing Operations Location: Connecticut / Massachusetts Employment Type: Full-time Travel: Multi-site; regular travel across assigned facilities About the Role We are seeking an experienced Director of Care to provide clinical leadership and oversight across a portfolio of approximately 10 skilled nursing facilities (SNFs) throughout Connecticut and Massachusetts .
