JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Follow physician’s orders. Renders and/or assist with complete nursing care, treatments, dressings, and other physician orders. Transcribe physician’s orders promptly and accurately. Review patient records daily to assure accuracy and completeness. Document comprehensive and complete nursing notes. Visit each resident daily to evaluate resident needs. Maintain accurate awareness of residents on alert list. Document and report any unusual or significant findings; contact the physician when necessary. Administer medication according to procedure. Maintain an accurate and safe control of all drugs for which he/she is responsible. Initiate any and all emergency procedures as necessary. Demonstrate correct safety techniques. Respects and takes care of facility equipment and not wasteful of supplies. Contributes to effective communication and pleasant working conditions. Contribute to and promote positive resident and family relations. Complies with facility Privacy Practices Policies & Procedures, protecting residents’ individual health information. Continue growth and expand job knowledge. Demonstrate leadership skills. Other duties assigned by manager. Specific Requirements: Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such actions. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care. Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for providing quality care. Must possess accurate and comprehensive assessment skills. Must be familiar with drug actions and interactions. Must have knowledge of restorative nursing rehabilitation policies and utilizes appropriately. Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. Must be able to communicate effectively to appropriate personnel regarding emergency situations.
