We have telemetry capability to provide the highest possible level of care for our patient population. 8E is an acute care unit that cares for adult surgical patients- Endocrine surgery and benign Gynecologic surgery and Gynecologic Oncology patients. Also medicine hematology patients including blood cancers- leukemia, lymphoma, myeloma, blood diseases- aplastic anemia, immune and genetic diseases. All nurses on this unit receive required, hospital based training, competency check off for chemotherapy administration and EKG interpretation training. JOB SUMMARY A Licensed Practical Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates patients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RN’s, Administrator, and Director of Nursing. Communicates effectively with the supervising RN, scheduled visits planned and changes to the schedule on a weekly basis. Caseload is self-scheduled but communication of the clinician’s schedule to the nursing supervisor is essential. Communicates in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual patient’s needs. Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend. Notifies the supervising RN with requests for re-authorization of “more visits needed” prior to third party insurers authorized number of visits expiring. Suggests Community Services that may be available to the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process as requested. Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function. Participates in the growth of the office by being a willing preceptor for newly hired, same discipline employees. Maintains LPN licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the patients serviced. Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event’s occurrence. Responsibilities* 6C Medicine Pulmonary Unit 6C is a 32-bed unit that provides 24-hour nursing care to adult patients. 6C is the home floor for Internal Medicine. Total patient care is utilized as 6Cs nursing care delivery system. Within this system, RNs assume the role of Patient Care Coordinator responsible for planning patient care from admission to discharge. Types of patients treated on 6C include pulmonary patients present with compromise of the cardiopulmonary system. Many of the patient population on 6C are transferred to this unit from the adjacent Critical Care Medicine Unit. These patients can often be a challenge to your nursing skills in the areas of assessment and clinical intervention. Medicine patients are typically admitted with problems such as asthma, Wegners granuloma, COPD, pleural effusion, pulmonary embolus, pneumothorax, pneumonia, sarcoidosis and cystic fibrosis. Responsibilities* 8D Our Patient and Families 8D is a 20-bed, Progressive Critical Care Unit which provides care for critical and moderate care medical-surgical patients, as well as those patients who are in transition from ICU to general care and rehabilitation floors.
