Job Summary The Registered Nurse is responsible for utilizing the nursing process to provide comprehensive care appropriate to the age of the patients served in Wheeler 5, Adult Medical, General Surgery, Oncology Unit as directed by nursing management. Nursing care is provided in accordance with the stated philosophy and standards established by the Nursing Service Department, regulatory agencies and professional organizations. The Registered Nurse is accountable for the safety of nursing care they deliver or supervise. Job Summary Summary 8 and 12 hour shifts every other weekend and holidays Grade 130 The Patient Care Assistant II functions as a member of the patient care team, under the direction of the Registered Nurse (RN), and proficiently and consistently performs all aspects of the PCA role. The Patient Care Assistant II is responsible for supporting the RN with clinical tasks, including but not limited to blood glucose monitoring, completing EKGs, applying simple dressings, using aseptic technique and responding to medical emergency situations utilizing Heartsaver CPR and AED skills. In addition, the PCA II is responsible for responding to the needs of patients and family members and taking an active role in the efficient operation of the unit. The PCA II will also assure the practice of the Patient and Family Centered Care Model in an environment that exemplifies best practice. Does this position require Patient Care? Yes Essential Functions -PROVISION OF A SAFE ENVIRONMENT • Anticipates and identifies patient safety risks, in cooperation with licensed nurse and takes appropriate action to reduce and/or eliminate the risks. For example: maintains proper bed height, uses side rails and alarms appropriately, places call light within patient reach, provides frequent patient checks and assists patient with ambulation, as indicated. • Makes routine patient room rounds to address patient needs. • Follows and maintains restraint policies, under the supervision of a licensed nurse. Job Summary Reporting to the Director of Hospital Medicine, provides daily evaluation and management to inpatients. This work will be in collaboration with the hospitalist service physician or surgical attending physician, and any physician specialists involved with each patient. The supervising physician of record will be one of the hospitalist physicians for purposes of establishing a collaboration agreement for licensure and prescribing status as well as periodic performance reviews. A second supervising physician may be established for surgical cases. About The Role The Home Care Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes. This role includes conducting patient assessments, developing individualized care plans, educating patients and families, and coordinating care to achieve optimal health outcomes. The RN works collaboratively with interdisciplinary team members to ensure safe, effective, and patient-centered care while maintaining compliance with regulatory and professional standards. Key Responsibilities Perform comprehensive patient assessments in the home setting Develop and implement individualized care plans and goals Provide skilled nursing care and treatments according to physician orders Educate patients and families on disease management, medications, and care plans Monitor patient progress and adjust care plans as needed Document care accurately in the electronic medical record Collaborate with physicians, therapists, and care team members to coordinate care Maintain patient confidentiality and adhere to professional and regulatory standards Education Minimum Qualifications: Graduate of an accredited professional nursing program required Bachelor of Science in Nursing (BSN) preferred Experience Minimum two (2) years of medical/surgical nursing experience preferred Experience or knowledge in community health or home health nursing preferred Licensure And Certifications Current Registered Nurse (RN) license in Massachusetts required BLS Basic Life Support (Healthcare Provider) certification required Skills And Competencies Strong clinical nursing and patient assessment skills Excellent communication and patient education abilities Critical thinking and sound clinical judgment Effective time management and organizational skills Proficiency with electronic medical records and computer systems Commitment to maintaining patient confidentiality Ability to read, write, and communicate effectively in English Demonstrated customer service and patient-centered care skills Benefits Of The Weekend Baylor Program Consistent 3-day workweek schedule Work-life balance with weekdays free Opportunity to build strong patient relationships in a home setting We are proud to share that Newsweek named Emerson Health one of the nation’s top Home Health Agencies for 2026.
