Add key accountabilities here. Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. Assesses assigned case load of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Job Profile Summary ​This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Clinical Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient’s plan of care as directed. Provides professional nursing care to patients. Requires a nursing license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. A senior level role that requires advanced knowledge of job area typically obtained through advanced education and work experience. Typically responsible for: managing projects / processes, working independently with limited supervision, coaching and reviewing the work of lower level professionals, resolving difficult and sometimes complex problems. In the Cath Lab, our nurses work closely alongside physicians and Radiologic Technologists during cardiac and pulmonary interventional procedures. Procedures range from diagnostic right heart catheterizations and coronary angiography to emergent PCI's and mechanical circulatory support (MCS). During procedures nurses are responsible for the monitoring of the patient, circulating the procedure, and case documentation. This includes monitoring vital signs and hemodynamic waveforms, administration of moderate sedation, maintaining any other medications such as vasopressors and anticoagulation, monitoring ACT's, and assisting device activation and maintenance of pacers and MCS. As we continue expanding this innovative model to care for patients along a continuum of services, the Home Hospital Nurse Navigator plays a vital role in assessing and educating eligible patients and facilitating safe and timely transfers from our Emergency Departments and inpatient units. ________________________________________ Position Summary The Home Hospital Nurse Navigator serves as the first point of contact for patients and families transitioning into the Home Hospital program. This role is responsible for assessing, identifying, and educating eligible patients, completing clinical and safety screenings, and coordinating the transfer process in collaboration with the Home Hospital Admitting Provider, Case Management, Administrative Coordinators, and unit-based care teams.
