Responsibilities We are actively hiring for an Registered Nurse (RN) Endoscopy. Develop an individualized perioperative patient plan of care by implementing: P rotecting the patient from injury caused by extraneous objects and chemical, electrical, mechanical, and thermal sources. D emonstrating safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation. S peak ing up with safety concerns acting as the patients advocate. Handling specimens according to facility policy and procedures. Adher ing to standard precautions including the use of personal protective equipment. Complying with Universal Protocol and facility site validation and time-out policy C ommunicating the patient’s current status to relevant parties throughout the continuum of care. A dministering medications safely and correctly using the 8 rights of medication administration. P racticing and monitoring aseptic technique . Implement ing the plan of care including the following interventions: M anaging patient recovery from anesthesia – Moderate sedation and Monitored anesthesia care . Assess ing patient status at intervals in accordance with facility policies and procedures . P roviding nursing care and appropriate monitoring in accordance with policies and procedures . P erforming interventions to maintain the patient’s tissue perfusion at or above baseline levels. Ensuring scopes are prepared for use in accordance with policies and procedures and manufacturer’s instructions for use . Maintain ing endoscopy instruments and accessories in working order. Troubleshooting and recognizing malfunctions . P erforming interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems and fluid, electrolyte, and acid-base balances at or above baseline levels. P erforming interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. P erforming interventions to protect the patient from infection. A ssessing the knowledge level of the patient or designated support person. P roviding education regarding the expected psychosocial response, nutritional management, medication management, pain management, and expected responses to the procedure. I nvolving the patient or designated support person actively in decisions affecting his or her perioperative plan of care and the rehabilitation process. P rotecting the patient’s rights, dignity, and privacy. P roviding age-specific, culturally competent, ethical care within legal standards of practice . Evaluate the patient’s progress toward attaining outcomes. Transfer care as applicable. Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Delegate tasks according to the state Nurse Practice Act (if applicable), state board of nursing or professional registration requirements, state laws, and job descriptions . Document episode of care accurately, timely, completely, and legibly. Demonstrate effective communication and collaboration skills with members of the team and other stakeholders. Serve as a member of a multidisciplinary team in planning patient care. Participate in quality review and performance improvement projects. Participate in the performance appraisal process. Provides support and direction to families in accordance with the patient’s Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. • Takes referral information and triages for immediate response, dispatches the on-call nurse to do the evaluation/ admission. Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. • Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. • Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. • Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. • Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. • Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. • Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. • Participates in special projects and performs other duties as assigned.
