Main Function: Under the supervision of the Clinical Nurse Manager, performs duties in the care and treatment of patients on maintenance dialysis. We are seeking a LPN who has at least 1 year of experience, caring for patients on dialysis. Candidate must be licensed in New York and have current BCLS certification. Description Duties Description Coordinates, directs and provides professional nursing care to residents of the Nursing Home under the direction and supervision of the RN 2 and/or ADON/DON. Notes: 1. Qualifications Minimum Qualifications Candidates must possess a license and current registration as a registered professional nurse in New York State. Reassignment: Current New York State Department of Health St. Albans Veterans Home employee with permanent or contingent permanent status as a Registered Nurse 1, Grade 18. Transfer: Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service in a position eligible for transfer under Section 70.1 of the Civil Service Law, to a Registered Nurse 1, Grade 18. Posted salary range ($76,147 - $85,138) includes statutory hiring rate at step 4 and job rate for grade 18, plus a geographic pay differential of $35,000 in addition to $4,000 of Downstate Adjustment. Inconvenience pay program of $575 per year will be paid to employees who work four (4) hours or more between 6:00 p.m. and 6:00 a.m. Hours worked between 6:00 a.m. and 6:00 p.m. on an overtime basis are not included for Inconvenience Pay. Additional Shift differential of 7% for evening shift and 10% for Night shift. This position is part of the NYS HELPS program. No examination is required. 187686 Job Description Plans, directs, coordinates and implements programs. Evaluates patients’ status, instructs appropriate staff regarding significant techniques, and acts as a resource person to the health care team. This role performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care team. Job Responsibility Reviews all protocols to maintain quality control and patient safety, and to assist Physicians with patient’s compliance. Participates in quality meetings to discuss cases and review clinical outcomes. Meets the educational needs of families and other health care professionals concerning the care of the patients. Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
