A continuum of care is provided for patients undergoing kidney transplant as they are supported through their unique living related, cadaveric or paired exchange donation transplant process. The nursing staff is specifically trained to perform skills unique to this patient population, including peritoneal dialysis through manual as well as cycler methods. The nursing staff actively participates in delivery of specialized care for patients who qualify for cutting-edge pharmacologic research studies under the supervision of nephrologists, transplant surgeons and clinical pharmacologists. An on-site outpatient kidney transplant clinic provides ongoing episodic care to transplant patients. The unit also provides care for a wide variety of general medical/surgical patients with common diagnoses. We have the ability to provide remote telemetry monitoring for 8 beds as needed. This mix of patients provides a dynamic environment where nurses can meet career goals. Rehab: The Inpatient Rehabilitation/2 West unit is a 36-bed unit specializing in acute adult rehabilitation. The unit serves patients who have suffered a loss of function related to an illness or injury such as stroke, amputation, brain injury, orthopedic, spinal cord injury, cardiac conditions and many other diagnoses. Patients complete three hours of therapy daily, including physical, occupational and/or speech therapy. Each patient is followed by a physical medicine and rehabilitation physician. The nursing staff is trained to care for all patient populations and works collaboratively with the therapy staff to achieve our 81 percent discharge-to-home success rate. The unit is CARF accredited and upholds the highest standards of care for the rehabilitation patient population. General Surgery: The General Surgery/5 South unit is a fast-paced, 29-bed unit with all private rooms located on the fifth floor of The Christ Hospital’s south wing. The General Surgery unit cares for adults undergoing complex surgeries, including Whipple’s, DIEP flaps, colon resections, ostomy creations, bariatric weight loss, mastectomies, hernia repairs, head and neck resections, urological procedures, and multiple laparoscopic surgeries. Nurses on the unit are experts in post-surgical care and work closely with physicians, nurse practitioners, case managers, physical therapists, social workers and our wound and ostomy care team. A strong emphasis is placed on the utilization of the Enhanced Recovery Pathway to optimize the surgical patient’s physiologic function and to facilitate their recovery. The general surgery care team is specifically trained to provide care unique to this patient population through our Ostomy Care Associate training. Our team-oriented and caring peer environment is great for both the experienced and new graduate nurse. Joint and Spine/Musculoskeletal: The orthopedic/neurosurgery unit is a fast-paced, 58-bed unit consisting of all private rooms located in The Joint and Spine Center (JSC) on floors 4-5. It has state-of-the art technology including the Navicare Nurse Call system, in room computing, patient information and precaution monitors as well as texting communication. The unit primarily provides specialized care for patients undergoing orthopedic/neurosurgical procedures including total joint replacements, fracture repairs, spinal surgeries, craniotomies and intracranial hemorrhage patients. The unit may also receive medical/surgical overflow. The experience of caring for the diverse patient population on this unit provides staff with a solid clinical foundation in medical surgical nursing. Oncology: The Inpatient Oncology Services/4 West unit is a 27-bed unit. Patients may be admitted to the unit at any time during their journey through the cancer continuum, from initial diagnosis to surgery, chemotherapy, management of complications and terminal care. Patients with medical-surgical conditions and hematological disorders such as sickle cell crisis may also be admitted. Care on this unit is provided by a multidisciplinary team of registered nurses, patient care assistants, physicians, nurse practitioners, case managers, health unit coordinators, an oncology pharmacist, an educator, chaplains, nutritionists, social workers, physical therapists, occupational therapists, and respiratory therapists. Behavioral Health: The Behavioral Services/6 West unit is a 31-bed unit providing a full range of services and comprehensive care to adult and geriatric patients with behavioral health needs. Key Responsibilities: • To perform or assist the resident with completing Activities of Daily Living (ADL). • Respond to residents call lights to provide maximum comfort, safety and privacy. • Transport residents by ambulation or by wheelchair to areas of community (dining room, lobby and activities). • Report any significant findings/observations to nurse immediately. • Assist residents on admission, transfer and discharge to make a smooth transition. • Responsible for the safekeeping of residents’ personal possessions. • Keep resident rooms neat and safe. • Follow proper infection control techniques • Respect the rights of each resident, particularly privacy and confidentiality. • Re-teach resident to perform ADL skills. • Provide restorative care to prevent pressure sores, contractures, and complications. • Participate in Continuous Quality Improvement and ensures follow-up occurs. • Converse with resident and family; assist them on adjustment to the community. • Ensure adequate hydration and nutrition of resident; report deviations from norm to the Charge Nurse promptly. • Perform blood pressure, pulse, respirations, intake/output and weights accurately. Job Description The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. The RN utilizes knowledge derived from the principles of biological, physical, behavioral, social, and nursing sciences to assess, plan, implement, and evaluate patient care. All care is provided based on the concepts inherent in the model of care for TCH which promotes an on-going partnership between patients and families and the team of healthcare providers. The care is culturally based and age specific. The RN adheres to American Nursing Association (ANA) code of ethics and to the scope of practice described in the Ohio Nurse Practice Act. Responsibilities The Nursing Process Assessment - Demonstrates the nursing skills to complete a holistic assessment of the patient’s physical, psychosocial, spiritual, and education needs utilizing The Christ Hospital Professional Practice Model. Understands signs and symptoms and data related to the patient’s presenting needs and actual or potential nursing diagnosis. Recognizes and appropriately reports to the physician and charge RN pertinent abnormal data and changes in the patient’s condition. Utilizes appropriate handoff methods to collaborate with other healthcare team members to communicate pertinent data regarding patient’s physiological and psychological data. Assesses patient, family, and environment for safety risks, proactively seeks out and performs safety initiatives. Diagnosis and Planning - Utilizes collected data to establish and prioritize a list of actual, and potential, individualized patient problems and needs. Prepares, coordinates, and revises multidisciplinary plan of care in collaboration with the patient and family to be supportive of the TCH model of care/Patient and Family Centered care. For each problem a measurable goal is set. Communicates the plan of care utilizing SBAR and handoff processes with all team members. Establish priorities of care, discusses plan of care during shift report, and participates in care conferences. Initiates discharge planning upon admission, makes appropriate referrals consistent with the anticipated length of stay. Implementation – The method of providing evidence based, quality care is based on assessed needs and standards of care while adhering to hospital policy and procedures to achieve set goals. Performs and delegates nursing actions based on appropriate orders demonstrates critical thinking and accountability for appropriate implementation. Provides ongoing, individualized holistic patient and family education based on needs and plan of care in collaboration with the multidisciplinary team. Demonstrates critical thinking upon review of physician orders, making appropriate referrals, & contacts other health team professionals. Responds to emergency situations that require an immediate, controlled precision in a skillful manner. Implements measures to prevent contamination and/or transmission of disease to include the use of appropriate protective devices and equipment to prevent injury and maintain a safe environment. Evaluation – Continuously evaluates nursing practice related to the plan of care in relation to the standards of care established through evidence-based literature and standards established by professional organization that have been built into order sets in the electronic medical record, the individual plan of care, and patient outcomes and goals. Maintains continuity of care by continuous evaluation during ”handoff” report, rounding, and upon transfers to other specialty units and post care facilities. RN’s who were employed at TCH prior to January 2010, and have more than 20 years of experience as an RN are permitted to pursue their BSN on a voluntary basis. A MSN or DNP are not required but supersede the requirement for the BSN. YEARS OF EXPERIENCE: None required REQUIRED SKILLS AND KNOWLEDGE: The RN demonstrates the knowledge, abilities, and skills to provide age and culturally specific patient care and education. The RN effectively communicates with peers, utilizes appropriate channels of communication and maintains absolute confidentiality. The RN maintains competence and demonstrates evidence of continuing professional growth. LICENSES & CERTIFICATIONS: The RN holds a current licensure to practice nursing in Ohio. Membership in professional organizations is desirable. BLS certification is required prior to patient contact. ACLS is required within 6 months of hire in step-down and ICU areas. Certification in area of clinical specialty is expected within five years of eligibility.
