Clinical Operations Coordinator (LPN)

At SYNERGY HomeCare of Clifton–Secaucus, we are seeking a Clinical Operations Coordinator (LPN) to support client assessments, care documentation, and clinical coordination across our growing home care agency. This role is critical to ensuring timely client onboarding, high-quality care planning, and operational efficiency. You will work closely with leadership and a Registered Nurse (RN) to support assessments and ensure compliance with New Jersey regulations. This is an ideal opportunity for an LPN who enjoys field work, autonomy, and a flexible schedule, while still making a meaningful impact on patient care. Key Responsibilities: Client Assessments & Field Visits. Conduct in-home client assessments and gather detailed clinical and environmental information. Observe and document client condition, functional needs, and safety risks. Identify changes in condition and escalate appropriately. Care Plan & Clinical Support Prepare assessment documentation for RN review and approval. Assist with development and updates of care plans. Ensure all documentation is accurate, complete, and submitted in a timely manner. Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. To monitor the performance of CNAs/GNAs, nursing and non-licensed personnel, provide education and counseling, perform disciplinary action as necessary, and complete performance evaluations. Duties and Responsibilities Administrative Functions Direct the day-to-day functions of the CNAs/GNAs or other nursing personnel in accordance with current rules, regulations, and guidelines that govern the long-term care Center. Ensure that all assigned nursing personnel including CNAs/GNAs comply with the written policies and procedures established by this Center. Periodically review the department’s policies, procedure manuals, job descriptions, etc. Make recommendations for revisions. Provide discipline and evaluations of assigned CNAs/GNAs and other nursing personnel in accordance with current rules, regulations, and guidelines that govern the Center. Issue verbal and written disciplinary warnings to assigned CNAs/GNAs and other nursing personnel for violations of current rules, regulations, and guidelines of the Center. Meet with your assigned nursing personnel, including CNAs/GNAs, as well as support personnel, in planning the shifts’ services, programs, and activities. Ensure that the Nursing Service Procedures Manual is current and reflects the day-to-day nursing procedures performed in this Center. Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual. Make written and oral reports/recommendations concerning the activities of your shift, as required. Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained. Ensure that all nursing service personnel are in compliance with their respective job descriptions. Participate in the maintenance and implementation of the Center’s quality assurance program for the nursing service department. Participate in Center surveys or inspections made by authorized government agencies, as may be requested. Periodically review the resident’s written discharge plan. Participate in the updating of the resident’s written discharge plan, as required. Assist in planning the nursing services portion of the resident’s discharge plan, as necessary. Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies, as required. Admit, transfer, and discharge residents, as required. Complete accident/incident reports, as necessary. Provide information to the Billing Office as necessary to insure accurate resident billing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Education Must possess, at a minimum, a Nursing Degree from an accredited college or university, or graduate from an approved LPN/RN program. Experience Not applicable. On-the-job training provided. Specific Requirements Must possess a current, unencumbered, active license to practice as an RN or LPN in this state. Must be able to read, write, speak, and understand the English language. Must be able to evaluate the performance, initiate disciplinary actions, and prepare and complete performance evaluations for personnel. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Nurse Practitioner – NP – Infusion Center– Days We are seeking an experienced Family Nurse Practitioner (FNP) with oncology experience and pediatric experience (or a strong comfort level caring for pediatric patients) to provide patient-centered care across our Same-Day Access and Infusion Center. This role supports the full spectrum of infusion services, including both benign and oncology patients in adult and pediatric populations , and involves acute symptom management, assessment and treatment of therapy-related complications, care coordination, and close collaboration with a multidisciplinary care team. The ideal candidate will be comfortable managing a diverse patient population receiving a wide range of infusion therapies and will work closely with physicians, nurses, pharmacists, and other clinical team members to ensure high-quality, timely care. Prior experience with CAR-T therapy is preferred but not required. This is a full time day shift position working 4 days per week and there is also a rotating Saturday requirement. This position is located at NYP/Brooklyn Methodist Hospital.
