MDS Coordinator – RN or LPN The Springs at Rochester Hills is seeking an experienced and detail-oriented MDS Coordinator (RN or LPN) to join our skilled nursing team. This is an excellent opportunity for a knowledgeable nurse who understands the MDS process and is committed to accurate assessments, regulatory compliance, and exceptional resident care. Position Summary The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) and MDS process in accordance with federal and state regulations. This position works closely with nursing, therapy, dietary, social services, and other members of the interdisciplinary team to ensure assessments and care plans accurately reflect each resident’s needs. Inpatient Rehab Nursing Assistant

About the unit We are a 64 bed Inpatient Rehab Unit that sees a variety of patients who need to work on both their functional and medical problems. We treat ortho patients recovering from hip fractures, complex knee replacement, back surgery with neurologic involvement, multi-trauma, and other orthopedic issues. We also see neurology patients recovering from stroke, brain and spinal cord tumors, Traumatic Brain Injuries, Parkinson’s, MS, and any cancer diagnosis with functional decline. We also have patients come to our unit who had amputations and other complex medical/surgical issues with functional decline. Patients participate in 15 hours of therapy across the 7-day week. Nursing continues the interventions practiced by the patients in therapy sessions, to continue safe mobility. For many of our patients, Rehab is the key to getting back home with their families. This is a very rewarding unit to work on, as we all get to take part in the progress patients make towards their goal of getting home. UNIT DESCRIPTION: 8 North East and West CPCU makes up a 68-bed unit cardiac step-down unit that provides an intermediate level of care to patients that typically have a cardiac related diagnosis such as coronary artery disease (CAD), congestive heart failure (CHF), myocardial infarction (MI) and heart arrhythmias such as atrial fibrillation (A-fib) and ventricular tachycardia (V-Tach). We also provide pre and post procedure care to patients that have undergone cardiac procedures, such as catheterization/angioplasty, stents, ablations, and placement of pacemakers, and defibrillators.
