Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Able and willing to travel within branch/office coverage area. Purpose of Position: Under the direction of the Paraprofessional Team Leader the Certified Home Health Aide (CHHA) performs assigned duties in the patient’s home in a safe and considerate manner as prescribed by the case manager.
