Overview JOB DESCRIPTION – POSITION REQUIREMENTS JOB TITLE: Staff Nurse II CC/Department: 7010/Emergency Services Reports to: Administrative Director of Emergency Services JOB SUMMARY: The Staff Nurse is a professional caregiver who assumes responsibility and accountability for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at Seton Medical Center and Nurse Practice Act. Clearly communicates health, treatment and discharge information, ensuring the patient, family member or significant other understands all discharge and follow-up instructions. 10 Effectively performs procedures, such as initiation of IV catheters and fluids, medication administration, dressings, patient teaching, etc., within their scope of practice, and unit, department and Medical Center policies and procedures. 11 Effectively performs other nursing procedures/skills, such as cardiac monitoring, rhythm interpretation, defibrillation, monitoring of conscious sedation, ABGs etc., based completed specialty training and their scope of practice, and unit, department and Medical Center policies and procedures. 12 Appropriately recognizes and quickly responds to emergency medical situations. a) Accurately assesses “A-B-Cs.” b) Initiates appropriate emergency treatment, i.e., CPR, O2, etc. c) Communicates critical information to other team members. 13 Maintains all required credentials and training (CA RN license, BLS, ACLS, PALS, NIHSS, AB508). 14 Remains current on emergency treatments/practices/procedures through attendance at staff meetings, continuing education events, in-services and the review of professional literature. 15 Integrates/applies new knowledge from education and research into clinical practice. 16 Demonstrates knowledge of and competence in age-related patient care. 17 Consistently demonstrates knowledge of state and federal regulations, such as Title XXII and COBRA/EMTALA. A licensed professional nurse under the supervision of a Registered Nurse who provides nursing care and services to residents in a long-term care setting. Provides clinical data and observations to contribute to the nursing plan of care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient report and assignments with Nursing Assistants upon the beginning of assigned shift • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to observe the resident’s physical and emotional status • Collects clinical data and reports significant clinical findings according to policy • Performs manual and technical skills according to the scope of practice under the direction of a Registered Nurse • Records care information accurately, timely and concisely. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Job Description The University of California, San Francisco (UCSF) Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center seeks a full-time Advanced Practice Provider (APP) to treat mechanical circulatory support (MCS), heart transplant, and advanced heart failure patients, among others. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers, caring for patients with advanced heart failure across the continuum of care. Our Advanced Heart Failure & MCS programs are nationally known for cutting edge advanced heart failure care and support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term). The APP serves as primary provider in the hospital setting for these patients and will work closely with the Advanced Heart Failure Cardiologist, Cardiothoracic Surgery and Intensive Care teams along with nursing, case management, social workers and other multidisciplinary teams to evaluate, treat and manage this highly complex patient population in the Intensive Care and Acute Care Units. Job Description : EDUCATION: Other: Graduate of an accredited school of nursing OR RN license evidences completion of a RN program CERTIFICATION & LICENSURE: RN-Registered Nurse for the state where care is provided BLS-Basic Life Support Healthcare Provider TYPICAL EXPERIENCE: 2 years of recent relevant experience. SKILLS AND KNOWLEDGE: Demonstrated general knowledge and competence with regard to nursing theories, concepts and practices; medical terminology; anatomy; and physiology. Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations. Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people. Time management and organizational skills, including the ability to prioritize patient care, assignments and work within standardized policies, procedures, and practices to achieve objectives and meet time sensitive activities. Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook). Work independently as well as be part of the team, while accomplishing multiple interventions in an ever-changing environment. Ability to communicate and engage team members, patients and families in a variety of stressful situations. Use existing procedures and critical thinking to identify and solve routine or complex problems. Ensure the privacy of each patient’s protected health information (PHI). Build collaborative relationships with peers and other healthcare providers to achieve departmental and corporate objectives. Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
