Job Description Job Summary: The Patient Navigator-Licensed Vocational Nurse (LVN)-Cancer Clinic works under the supervision of a Registered Nurse (RN), Medical Doctor (MD) or Nurse Practitioner (NP) to provide developmentally appropriate and age-specific direct and indirect patient care in a fast-paced outpatient cancer clinic and infusion center serving patients eighteen years of age or older with suspected or confirmed malignancies or hematologic disorder(s). Performs designated procedures and treatments which require knowledge and technical expertise within the LVN scope of practice and in accordance with established policies, procedures, protocols, techniques, and guidelines. Recognizes and reports signs and symptoms of complications and/or allergic reactions immediately to the RN, MD or NP. Exhibits mission-focused thinking and understands the sensitivity of cancer diagnosis and expresses kindness when communicating with patients facing the unknown of a serious critical illness. Orients patients to the clinic routine and prepares patients for examination. Assists MD or NP with examination and procedures as requested. Ensures insurance authorizations, tests results, appropriate coding, and other required information is available for the provider for review. Follows-up on designated case management and navigation activities as requested by the RN, MD or NP. Demonstrates flexibility and willingly floats to other areas as assigned. Performs other duties as needed. Education and Experience: Graduate of a school of vocational nursing required. Minimum six months experience as an LVN or one year of experience as a Medical Assistant (MA) in a physician back-office setting required. Minimum one year clinical experience in oncology or hematology nursing or infusion center preferred. Knowledge and Skills: Demonstrates solid knowledge of oncology/hematology disease process and treatments. Your Role on the Team The position of Nurse Practitioner II 90% in the Department of Medicine, Division of Hospital Medicine & Palliative Medicine, requires a highly competent, independent yet team-oriented individual who will be immersed in Hospital Medicine. Under the supervision of Anuj Ohri, MD and guided by the applicable policies and procedures, the Nurse Practitioner will participate in the comprehensive care of patients in the ambulatory or inpatient settings and provide multi-disciplinary care coordination, treatment, education, monitoring, and communication with patients and referring physicians. A Nurse Practitioner is an Advanced Practice Provider (APP) who possesses the advanced education, knowledge, clinical skills and experience necessary to provide advanced nursing care in their area of specialization. This Position May Perform Functions as appropriate to their “scope of practice” (In general, the “scope of practice” includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per their approved standardized procedure). Familiarity with state statute for their licenses is necessary. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Certified Nursing Assistant (CNA) CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company Standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.
