MDS Coordinator - 3 DAYS A WEEK Job Summary: Mountain Ridge Rehabilitation and Healthcare Center, a Long-Term Care facility in Swannanoa, NC, is seeking a dedicated and experienced MDS Coordinator to join our team. As an MDS Coordinator, you will be responsible for coordinating, managing, and monitoring the written plan of care for each resident, ensuring that their needs are accurately addressed and goals are achieved. Key Responsibilities: • Keep abreast of current federal and state regulations, as well as professional standards • Assist with the development of comprehensive care plans for residents in coordination with the MDS • Coordinate, manage, and monitor the written plan of care for each resident • Assist nursing management with the coordination, management, and review of nurse's notes • Monitor resident status changes to ensure timely nursing or clinical involvement • Assure MDS and support documentation are accurate and meet regulatory and auditor requirements • Perform regular audits of documentation to ensure accuracy • Assist with the discharge process • Complete necessary forms and reports • Maintain strict confidentiality regarding sensitive health information • Report hazardous conditions, damaged equipment, and supply issues to appropriate persons Requirements: • Current State License as a Nurse • C.P.R. Certified • One year experience as a Nurse in a long-term care setting (preferred) Why work with us? Summary: The Clinical Nursing Assistant is responsible for coordinating and delivering patient care utilizing the nursing process under the direction of a Registered Nurse in a hospital setting. Responsibilities: Assist physicians and other health care providers with routine examinations and procedures, as well as procurement of equipment and supplies during emergency situations. Observes, collects, and documents patient information obtained while providing care and promptly report significant changes in the patient's condition to a registered nurse or physician. Assists patients and their families during admission to the clinical area, orientation to the hospital/clinic/home health experience, and discharge . Provides care/ treatments to patients as outlined in plan of car e including, but not limited to: Monitoring of vital signs Measurement of height and weight Specimen collection Skin preps/scrubs such as chlorohexidine (CHG) bathing Catheter care Capillary blood glucose checks Simple wound care Range-of-motion exercises Drain care Assist patients with routine activities of daily living (ADL) such as bathing, dressing, feeding and assisting with ambulation as outlined in plan of care Maintains an effective clinical work environment including but not limited to cleaning the work area, changing linens, maintaining and monitoring suppl y levels, and transporting patients / specimens throughout hospital. Ensures competency in all required skills and completes competency assessment annually per the documented competency checklist. Adheres to documented policies and processes as well as performing other related duties as assigned.
