Job Summary: The Registered Nurse (RN) provides holistic patient care as a key member of the Mobile Health team. The RN performs essential clinical tasks in a mobile healthcare setting, ensuring efficient, high-quality patient care across diverse and often underserved communities. This position combines clinical expertise with adaptability, delivering care in non-traditional environments including mobile clinics, shelters, agricultural sites, correctional settings, and community-based outreach events. The RN provides care across multiple Mobile Health service lines, including Mobile Primary Care, the Center for Addictions Triage and Treatment (CATT), Low Acuity Transitional Services (LATS)/Medical Respite, the Farmworker Outreach Program (FOP), Correctional health services, and community-based outreach programs. Essential Duties and Responsibilities: • Conduct comprehensive assessments of patients, including medical history, current health • Conduct comprehensive patient assessments, including medical history, current health concerns, and vital signs in mobile and field-based settings • Identify patients with significant health risks, including those related to chronic conditions, substance use, and social determinants of health • Provide patient education on diagnosis, treatment plans, medications, infection control, and preventative care tailored to community needs • Support chronic disease management (e.g., diabetes, hypertension, asthma), including lifestyle education and medication adherence • Triage walk-in patients and coordinate care in collaboration with providers • Administer medications (oral, IM, IV) under provider supervision as appropriate • Perform point-of-care testing and laboratory procedures • Manage high-risk patients and coordinate follow-up care across multiple service locations • Maintain infection control standards within mobile and field environments • Utilize telehealth tools and remote consultation as needed • Coordinate patient flow and assist with mobile clinic setup and breakdown • Provide group and community-based health education • Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). • Perform other duties as assigned. • Valid driver’s license, reliable transportation, safe driving record and insurance coverage required. Clinical Role & Operations • Lead and support clinic flow across mobile sites, ensuring efficient and safe patient care delivery • Delegate tasks appropriately to LPNs, MAs, and support staff in alignment with scope of practice • Coordinate care across multiple service lines and partner organizations • Adapt clinical workflows based on site type (respite, outreach, correctional, event-based care) • Support operational readiness, including supplies, equipment, and site setup HIPAA Requirements: The Registered Nurse – Mobile Health has access to PHI to create and maintain an accurate and up-to-date health record. Applying the minimum necessary standard of HIPAA, the designated record sets to which this employee will have access include all sections of the dental and medical record, patient demographic information in the practice management system, incoming records, reports, results, consultations, etc. The [position title] should read the content of these records only to the extent needed to accomplish the assigned task (e.g. filing or disclosure). Knowledge, Skills and Abilities Required: • Strong commitment to trauma-informed care, cultural humility, harm reduction, and health equity • Ability to provide compassionate, high-quality care to vulnerable and underserved populations • Experience working with patients experiencing homelessness, substance use disorders, and complex medical and social needs • Strong wound care, vaccine administration, infection control, and acute care skills • Ability to work effectively in non-traditional, high-volume, and mobile environments • Strong coordination and communication skills across multidisciplinary teams and partner organizations • Proficiency in medical documentation and electronic health records (Epic preferred) • Bilingual English/Spanish preferred Education and Experience Required: • Current Oregon RN License. • Experience in community health, mobile health, or working with underserved populations preferred. Working Environment/Physical Hazards: • Work in a variety of environments, including outdoor settings, community centers, and mobile units. • Potential exposure to bloodborne pathogens and hazardous materials. • Adherence to strict infection control measures within mobile units. • Comfortable working in fast-paced, dynamic environments • The ideal RN will be comfortable utilizing their full scope of practice including assessment, planning, implementation, supervision and delegation of appropriate tasks to licensed and unlicensed team members (LPN's, CNA's, and ancillary staff) as outlined by the Oregon Nurse Practice Act Equipment Used: • Medical equipment (e.g., autoclave, pulse oximeter, glucometer) and mobile-specific equipment as required for remote care. • Office equipment (e.g., laptops, mobile devices) for documentation and communication. Contact a supervisor immediately with discrepancies.Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days.Notify supervisor when supply is less than 2 days and medicine hasn’t been received.Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR. Notifies pharmacy and DH&W of any discrepancies.Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications.Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used.Responsible for weekly inspections of all medications to ensure all expired medications are removed.Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies.Responsible for removing all discontinued medication from med cart and giving it to the supervisor.Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc.Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights.Document appropriate information in the daily log as required and notify supervisor of all concerns.Answer calls and assist residents with needs as assigned.Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. The Recuperative Care Program (RCP) is a new program in partnership with Washington County Supportive Housing Services and Greater Good Northwest to provide acute and post-acute care specifically for people experiencing homelessness who are too ill or frail to recover from an illness or injury on the streets or in emergency shelter, but who do not require hospital-level care or skilled nursing services. This position will be based out of the VG 7th Ave clinic and will require travel to the RCP program site in Hillsboro and may require travel to local hospitals for discharge planning. Essential Duties and Responsibilities: · Screen referrals from hospitals and coordinate with discharge planners to assess patient acuity, program requirements, and bed availability to determine appropriateness for RCP admission. · Identify potential clinical and supportive service needs while within respite program. · Proactively communicate with RCP participant’s primary care team (VG or external) to facilitate early post-hospital contact. · Obtain required consents and releases for coordination of care. · Develop care plan based on patient goals and the reason for admission to RCP. · Gather information for a clinical care plan including physical and psychological history and assessment; list of current medications, dosages, and allergies; vital signs; other pertinent information, such as laboratory or other test results. · Identify discharge indicators and develop a discharge plan with patient. · Participate in and facilitate multi-disciplinary care coordination meetings with appropriate providers (e.g., PCP, home health, pharmacies, specialty care). · Coordinate with shelter case management to address SDOH and other non-clinical needs, · Provide shelter staff education and support around basic medical knowledge as relevant for evening and weekend supervision of RCP residents. · Huddle daily with RCP team to create and implement daily action plans, performing transition of care visits for patients being discharged from hospitals, following up with outside provider recommendations after outpatient visits and ensuring that treatment recommendations and medication changes are acknowledged and actualized. · Facilitate patient access to obtain durable medical equipment (DME), wound care, oxygen, and incontinence supplies (as needed). · Provide patient education and health coaching on topics including diabetes, hypertension, and other chronic diseases, including assisting patients with self-management goal setting. · Educate patients on how to utilize after-hours number or daytime nurse triage to address immediate clinical needs and provide guidelines for when to contact RCP team or PCP vs. seeking emergency department care. · Log vital signs and monitor for changes in health status as indicated by the care plan. · Administer pre- and post-procedural or operative care, including minor wound care per discharge instructions, monitor for changes in health, and recommend follow-up services as needed. · Support self-administration of medications and patient progress towards self-management of medications. PURPOSE AND SCOPE: The registered professional nurse Home Therapies RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). Job Summary: The Call Center Advice Nurse (CCAN) provides clinical advice and support over the phone to patients calling our call center.
