We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies. Provides supervision and training for Licensed Practical Nurses and/or less experienced. Provides immediate first aid with referral to outside emergency services or private physician. Assesses, plans and evaluates nursing care delivered to patients/residents requiring long-term and/or rehabilitation care. Delivers nursing care to patients/residents requiring long-term and/or rehabilitative care . Implements the patient/resident plan of care and evaluates the patient/resident response. Directs and supervises care provided by other nursing personnel. Provides input in the formulation and evaluation of standards of care. Maintains knowledge of necessary documentation requirements . Maintains knowledge of equipment set-up, maintenance and use (i.e. monitors, infusion devices, drain devices, etc.). Maintains confidentiality and compliance with patient/resident rights, relating to all patient/resident/personnel information. Provides patient/resident/family/caregiver education as appropriate and/or directed. Conducts self in a professional manner in compliance with unit and facility policies . Works rotating shifts, holidays and weekends as scheduled. Registered Nurse ( RN ) Required Education and Experience: Must be a graduate of a basic education program in professional nursing. A minimum of one year medical/surgical nursing experience in an acute care (hospital) setting or long-term care setting (acute/critical care is preferred). Must possess a current Registered Nurse ( RN ) license in state(s) of practice. Must have a reliable source of transportation. The Registered Nurse ( RN ) is responsible to plan and deliver nursing care to patients/residents requiring long-term and/or rehabilitative care.

We are seeking a dedicated and compassionate Staff Nurse to join our team at a group home serving individuals with intellectual and developmental disabilities. The primary responsibility of this role is to provide direct care including G-tube feeding, support with Activities of Daily Living (ADLs) and overall health monitoring to ensure the safety and well-being of our individuals. Responsibilities: Administer G-tube feedings in accordance with prescribed care plans Assist individuals with ADLs including hygiene, mobility, and personal care Ensure accurate documentation in compliance with NJ DDD Assist with other household duties such as cooking, cleaning, and community outings as part of a team-oriented approach Qualifications: Graduate of an accredited nursing program Valid RN or LPN license in the State of New Jersey Proficient in G-tube administration and related care Valid NJ Driver’s License with a clean driving record CPR/First Aid certification required Experience in community health or home care settings preferred Shifts available: Monday to Friday 4pm-8pm, Saturday and Sunday 9am-1pm and 4pm-8pm.
